Exalt Health is an acute rehabilitation hospital that offers an intensive rehabilitation program to patients with a variety of complex needs like stroke, spinal cord injury, and more.

About the Job: We are looking for a dedicated Case Manager to work closely with physicians to oversee individual patient programs, ensuring their progress through care and achieving positive outcomes. This role involves coordinating treatment plans, connecting patients with resources, and facilitating smooth transitions to post-discharge care.

Responsibilities:

Comprehensive Assessment: Perform detailed assessments to identify patient needs and support systems
Collaborative Care Planning: Develop personalized care plans with the team
Resource Navigation: Connect patients with community resources
Discharge Planning: Coordinate safe transitions to the next level of care
Insurance and Authorization Management: Liaise with insurance providers for treatment authorizations
Advocacy and Support: Advocate for patients and families
Documentation and Reporting: Maintain accurate patient records

Requirements:

Knowledge, Skills, and Abilities: Relevant licenses or certifications required
Education, Training, and Experience: Healthcare experience preferred
Skills and Abilities: Excellent attention to detail and problem-solving skills
