CASE MANAGER
The Case Manageris responsible for the overall case management of youth in Creighton Oaks RTC. The Case Manager completes intakes and collects the educational portfolios from caseworkers. The Case Manager maintains client files.

The Case Manager is responsible for ensuring youth's individual needs are being met while residing in Creighton Oaks RTC and updating the youths Caseworkers on the youth's status weekly and as needed.


Education:
Must have a Bachelor's degree in psychology, sociology, social work, criminal justice or related field. Master's degree preferred.

Experience:
One year experience in residential child care setting.


Job Functions:

  • Completes screenings in a professional manner and approves placement of new youth in coordination with the Program Director.
  • Schedules and completes intakes during work hours. Assist the caseworker and youth in paperwork and understanding the Creighton Oaks policies and procedures.
  • Conducts assessments, service and treatment plans as required by DFPS licensing.
  • Makes appointments for youth medical, dental and mental health services.
  • Maintains a youth file for each youth assigned and ensures all paperwork is complete.
  • Maintains an educational portfolio for each youth and participates in youths educational meetings.
  • Develops a positive relationship with and calls caseworkers weekly for updates on discharge plans and/or to discuss any accomplishments or concerns with youth. When possible, allows youth to talk with caseworker.
  • Liason between Creighton Oaks staff and caseworkers when there are concerns or needs for a youth in Creighton Oaks RTC. Assists in deescalating youth and/or caseworkers and provides a supportive approach to difficult situations.
  • Works closely with the medical care worker to ensure all youths needs are being met.
  • Conducts discharges of youth during working hours and provides caseworker with copies of all discharge paperwork and school portfolio.
  • Attends Creighton Oaks staffings and provides pertinent information while advocating for the youths best interest. Demonstrates a professional attitude while working with others in staffings.
  • Attends weekly administration meetings, general staff meetings and other meetings required. Provides professional feedback in meetings and maintains a professional and positive relationship with Creighton Oaks staff and administration.
  • Participates on the treatment team.
  • Reports suspected child abuse to DFPS Licensing and the Program Director.
  • Follows all minimum standard rules and Creighton Oaks Policy and Procedures.

Pay:
$ $25.00 per hour


Schedule:

  • 8 hour shift
  • Monday to Friday
  • On call
