Villa Toscana Memory Care Community is located in Mountain View, California. We cater exclusively to those with memory care needs. Our beautiful community is close in vicinity to various local restaurants, shopping, and local activities.

JOB SUMMARY:


  • This is a Supervisory position. The person in this position ensures that care and services for all residents is provided in a consistent and efficient manner. The Care Coordinator maintains a compassionate and caring attitude toward residents, resident's families, prospective residents and staff under all kinds of circumstances and conditions. This person provides the overall direction and coordination of the Assisted Living Program to ensure compliance to policies, procedures and standards set forth by Crescent Oaks as well as State, Local, Federal laws and regulations connected to the operation and/or licensure of the community and its staff.

Qualifications:


  • Above all, The Memory Care Coordinator must maintain a compassionate and caring attitude towards Residents of the community.
  • This position requires clear written and verbal communication skills.
  • This position also requires the person be able to read, write, understand and communicate in English
  • It will be mandatory within the first (7) days of employment, for this person to have a Health Certification signed by a physician. This certification must verify that the Memory Care Coordinator has no communicable diseases, open lesions, or any other health problem, which could interfere with individual's job performance.
  • The memory Care coordinator understands that Crescent Oaks operates as a Drug Free Workplace per Policies set forth in the Company Handbook.
  • The memory Care coordinator will dress appropriately, according to the requirements set forth by their position.
  • The memory Care coordinator must be available to lift 25 pounds.
  • Must have High School Diploma, College Degree or Some College and/or Care Coordinator Certification.
  • Valid CA Driver's License, with DMV Printout
  • Must be efficient in Microsoft Office Suite or similar and have the ability to create calendar of events, invitations, charts, graphs, reports as required by the position.

OUR REQUIREMENTS:

Due to our Licensing, we are required to have a clear LiveScan clean of convictions or active arrests for all of our employees in order to be hired.

  • We will pay for this and help you set up the appointment._
  • If you have had a LiveScan in the past 2 years please let us know._

Must have a TB test and we ask that you get a flu shot.

  • Good physical health shall be verified by a health screening performed by a physician not more than (6) months prior to or (7) days after employment. This certification must verify that the Caregiver has no communicable diseases, open lesions, or any other health problem, which could interfere with the individual's job performance_

Must have current Adult First Aid card at the time of employment and maintain throughout the length of employment

  • Must have dependable transportation and be able to work varying hours as needed, nights, holidays, and weekends.
  • This job may require the employee to lift in excess of 25 pounds on occasion, which is a qualification for this position. It may also require the mandatory use of a gait belt and the wearing of a back brace for more difficult residents.
  • You will be expected to report to work on time to give the Administrator adequate notice if you are unable to show for work: at least four (4) hours' notice.
  • Participate in mandatory ongoing education and inservice training.
