$65K range - based on experience_

Qualifications/Requirements:

Must be an NJ certified Social Worker
Must have experience as a Social Worker
Must have long term care/nursing home or subacute rehab experience
Experience with Point Click Care preferred
Must have experience with assessments, Care Plans and Discharger Planning
Strong written and verbal communication skills

Social Worker Job Description:

Assist new residents and their family in the initial adjustment phase, as well as in the ongoing transition to institutional living
Provide information to the resident, and/or family member, on residents' rights and responsibilities
