Healthy Lifestyle Project Manager - Milwaukee, United States - Sixteenth Street Community Health Centers
Description
Coordinate and lead Healthy Lifestyle programming, initiatives, and community events for patients and community members.- Collaborate with internal and external project partners and advisory groups to advance the objectives of the Department of Environmental Health & Community Wellness.
- Cultivate and maintain interdepartmental relationships, evaluate, and implement opportunities for interdepartmental collaboration.
- Cultivate and maintain relationships with key public, private and nonprofit sector individuals and groups whose work supports healthy lifestyles and consider opportunities for collaborations.
- Coordinate external and/or internal stakeholders to build and lead project teams.
- Assist in the identification of funding sources, development of grant proposals, completion of grant reports, and other administrative duties related to grant management.
- Track grant/project deliverables and manage project teams to ensure project goals are achieved.
- Recruit, train and supervise AmeriCorps members, volunteers, and other part time staff as needed.
- Prepare outcomebased reports (incentives and expenses) for FP and Latinos Por La Salud (LPLS) programs.
- Participate in the Department's public relations through media events, written publications, and/or oral presentations.
- Assist LPLS trainings and projects (facilitation, recruitment, community projects logistics and members general support and motivation).
- Train, orient, supervise Comprehensive Community Care (CCS) Rehabilitation Worker
- Assist Community Projects and Healthy Schools programs when needed.
- Collaborate and coordinate activities and programing with other SSCHC departments involved with obesity (diabetes, environmental health, and behavioral health).
- Assist with other program initiatives and perform other responsibilities as needed under the direction of the Director of Environmental Health and Community Wellness.
- Comply with SSCHC TB Control Plan, including PPD testing.
- Other duties as assigned.
Qualifications:
- Effective bilingual written and oral communication skills (Spanish/English).
- Bachelor's degree in a health, education, nutrition, or communications related field strongly preferred; consideration will be made for a suitable combination of previous academic and professional experience.
- Ability to take initiative and provide effective leadership.
- Experience working for a communitybased and/or healthrelated agency or program.
- Ability to relate well to and communicate well with people of various ages and from diverse ethnic and cultural backgrounds.
- Excellent attention to detail and strong organizational skills.
- Ability to work occasional evening and weekend hours.
- Valid Wisconsin driver's license, use of an automobile, and current auto insurance.
