    Summary:


    The Home Care Manager oversees the day-to-day operations of the PACE Home Care Program to ensure regulatory compliance and quality while maintaining an optimal level of independence for participants; acts as a member of the interdisciplinary team; is responsible for assessment of participant health needs and contributing to the plan of care.

    Essential Duties and


    Responsibilities:
    Oversees or performs in-home safety evaluations, initial comprehensive, semi-annual and change of condition assessments of participants in their home environment to determine their individual needs and coordinate plan of care with the participant, caregiver, and interdisciplinary team

    Assures that home care caregivers provide supportive and restorative nursing care to participants as ordered, based on the plan of care, participant needs and document timely and appropriately in the medical record

    Assures timely education and training for participants, caregivers, and families on an individual or group basis as needed

    Manages direct report staff, including but not limited to:

    Hires, selects, directs, supervises, and evaluates the performance of direct reports. Handles discipline and termination, if necessary, of associates in accordance with policy

    Oversees the daily workflow of the department

    Ensures that the job responsibilities, authorities, and accountability of all direct reports are defined and understood

    Assists in providing in-service education for the New Horizons PACE employees; Responsible for the coordination of personal care activities of all participants in the home

    Manages and prioritizes utilization of homecare services provided and fiscal responsibility for departmental budgetary requirements

    Participates in the after-hours and weekend on-call rotation and coordinates 24-hour care delivery

    Maintains up-to-date and appropriate documentation in the participant's electronic medical record

    Participates in the interdisciplinary team meetings, communicating participant changes, collaborating on care planning decisions and coordination of 24-hour care delivery

    Qualifications, Knowledge, Skills, and Abilities:

    • Active state licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN)
    • (3) years nursing management, preferably in home and community health.
    • Medical clearance for communicable diseases and up-to-date immunizations, before having direct participant contact.
    • Ability to assess, plan care for and provide service to frail elderly enrollees according to assigned responsibilities and PACE Program standards; Knowledge of home care services nursing principles and practices with reference to the elderly; Broad knowledge base of health care problems of the frail elderly; Skilled in preparing and maintaining records and written reports and responding to correspondence; Ability to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE participants' care; Excellent oral and written communication skills; Strong interpersonal skills with ability to build relationships; Effective organizational skills with attention to detail and time management skills; Strong analytical and problem-solving skills; Strong supervisory and leadership skills; Proficient with Microsoft Office Suite or related software; Proficient with Electronic Medical Records.
    Physical Activities and Demands;

    Working Environment:


    Prolonged periods sitting, standing, and walking; Ability to squat, crouch, bend, kneel, and crawl occasionally; Ability to push, pull, reach above shoulders occasionally; Ability to frequently lift/carry up to 10 pounds, occasionally lift/carry up to 25 pounds using appropriate body mechanics with reasonable accommodation if needed; Frequently use hands for simple and firm grasping and fine manipulation; Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus; Exposure to bodily fluids, bloodborne pathogens, and/or other infectious illnesses common in a healthcare environment; Exposure to hazardous chemicals such as disinfectants, cleansers, soaps, etc.; Exposure to fluctuating indoor temperatures; Noise level in the work environment is usually moderate to loud; Subject to unpleasant odors; Potential exposure to verbal aggression by participant; Up to 75% of the time in participant homes, balance at New Horizons PACE site.

    Additional Information:
    Oversee daily operations for home care withing a PACE program. Must be a RN or LPN. Completes in home assessments and supervises home care aides.
