The
Licensed Practical
Nurse (LPN) in Hospice is responsible for providing quality patient care and performing technical skilled care in the patient's home.


  • Performs skilled nursing services under the supervision of a staff RN daily.
  • Administers prescribed medications and treatments as permitted by state regulations.
  • Assists the physician and/or staff RN in performing specialized procedures as needed.
  • Assists the patient with daily activities and encourages appropriate selfcare daily.
  • Reports to the patient's physician and other appropriate personnel regarding pertinent changes in the patient's condition as needed.
  • Performs complex wound care if inservice is documented for specific procedure daily or as needed.

Formal Education:
Associate Degree or certification equivalent

License, Registration, and / or Certification Requirement: Yes


_ License Requirements_

  • Unrestricted licensure in state of practice.
  • Current CPR certification required.
  • Current driver's license and vehicle insurance, and access to a dependable vehicle or public transportation.
