    Description

    SUMMARY

    : The Case Manager will assist the A/CDP counselors, Coordinator, and Program Director with meeting A/CDP client's needs. This position will also serve as the Peer Advocate by providing support, education, and advocacy to clients and coordinate services with other agencies. Will help expedite and coordinate access to treatment and recovery services, link clients to resources in the community, help with scheduling, collecting data and support the needs of clients.

    ESSENTIAL DUTIES AND RESPONSIBILITIES OF A CASE MANAGER:

    · Work as part of the A/CDP team to provide quality client care.

    · Coordinating client care by helping counselors gather and process referral paperwork needed for referrals to other agencies for inpatient treatment, detoxes or halfway houses.

    · Ensure the smooth transition of referring clients to all required internal and external services necessary to help client with their treatment needs according to their treatment plan.

    · Work with counselors to ensure all New York State Office of Alcohol Abuse Services (OASAS) paperwork requirements are met. Case Manager will help maintain case records and provide quality assurance of records according to OASAS part 822 regulations.

    · Be able to extract reporting data from the RPMS system.

    · Use the RPMS Scheduling Program to schedule clients, when needed.

    · Work with counselors to ensure quality case management.

    · Document clients contact in the Behavior Health System (electronic health record) according to New York State Office of Alcohol Abuse Services (OASAS) requirements.

    · Provide quality assurance for counselor's files according to New York State Office of Alcohol Abuse Services (OASAS) regulations.

    · Familiarity and involvement with monthly and yearly reporting requirements for NYS OASAS and IHS RPMS.

    · Monitors, develops, and maintains data input for required programs and can work with accounting and auditors on data requirements.

    · Become familiar with yearly County, State and Federal surveys regarding client care, capacity, staff workforce needs, and program development.

    · Work with program supervisors to train staff on various procedures of Tribal processes.

    · Assist with continued update and development of A/CDP Outpatient policy and procedures.

    · Assist in maintaining budget data for A/CDP Outpatient

    · Assist in maintaining counselor caseloads, group lists, active client roster, and organizing the digitalizing process for client files.

    · Attends and is involved with various committees such as GPRA, Compliance, and any other committees that are necessary.

    · Work with the Department of Health to maintain proper NARCAN data and reporting requirements.

    · Attend Community events when required.

    ESSENTIAL DUTIES AND RESPONSIBILITIES OF A PEER ADVOCATE:

    · Provide non-clinical crisis support when needed.

    · Provide transportation to and from on-site appointments for clients when needed.

    · Provide advocacy, outreach and engagement of clients and their families in forming a supportive, recovery network.

    · Help clients and families to overcome barriers, ambivalence, and challenges in services.

    · Provide transitional support and self-help tools to assist individuals as they move toward care and recovery.

    · Partner effectively with other programs and staff to insure effective connections.

    · Network with other agencies, groups, and organizations within the addictions and human services fields.

    · Link Clients to resources in the community.

    SUPERVISORY RESPONSIBILITIES: There are no supervisor requirements for this position.

    QUALIFICATION REQUIREMENTS:

    Maturity and ability to handle delicate role issues and to respond to crisis. Be willing to work with a treatment team. Excellent oral and written communication skills. Computer skills are required. Understand the disease concept of alcoholism/chemical dependency. Familiarity with Akwesasne Community an asset.

    EDUCATION/EXPERIENCE:

    · Associates Degree preferred and

    · must be willing to work towards becoming a Certified Recovery Peer Advocate (CRPA).

    · Two years' work experience/training in addiction field or related area an asset.

    REASONING ABILITY: The ability to communicate effectively verbally and in writing as appropriate for the audience so others will understand. Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times.
