    Position: Case Manager

    Job Description:

    We are looking for a dedicated and caring Case Manager to become part of our team. The role involves assisting individuals and families in accessing social services, addressing challenges like homelessness, mental health issues, substance abuse, and domestic violence.

    Responsibilities:

    • Conduct thorough assessments and create personalized care plans
    • Link clients to suitable resources for housing, healthcare, counseling, and employment training
    • Track clients' progress and adjust plans when necessary
    • Collaborate with professionals from various fields
    • Advocate for clients' rights and service accessibility
    • Maintain accurate case records
    • Offer crisis intervention and support during emergencies
    • Participate in case discussions and team gatherings

    Qualifications:

    • Bachelor's degree in social work, psychology, or a related field (Master's degree is a plus)
    • Prior experience in case management or social services
    • Effective communication and interpersonal abilities
    • Capability to engage with diverse groups
    • Knowledge of community resources and social service systems
    • Empathy, patience, and a non-judgmental approach
    • Ability to work both independently and collaboratively
    • Proficient in computer skills and case notes documentation
