- Coordinate with an interdisciplinary team to deliver comprehensive plan of care to provide added support to patients and families.
- This position is unique in providing an opportunity to have a tremendous impact on patient and family quality of life at a crucial time.
- This is an ideal opportunity for someone who enjoys working closely with a team (our teams include patient care techs, medical directors with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers).
- NYS RN Licensure
- ASN required, BSN preferred
- 1-2 years recent acute care experience, Hospice experience highly preferable
- Need is rotating shift 12 hour nights and days. IDEAL candidate will be willing to work both night shift and day shift (12 hour shifts). Current needs are mainly for 12 hour night shift coverage.
- Excellent communication skills. Must be able to communicate effectively with patients, physicians, and peers.

$33.10-$45.75_
_Pay is based on experience, skills, and education.
Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
The pay range may also vary within the stated range based on location._PER DIEM AGREEMENT WITH RATES OUTSIDE OF ABOVE EXPERIENCE-BASED PAY RANGE*
