QUALIFICATIONS
Knowledge of employment and disability related laws and the rights of people with disabilities.

Knowledge of programs and services available in the community.

Must possess excellent "people skills".

Self-starter with the ability to organize, prioritize and complete tasks in a timely, efficient manner.

Must be able to function as a team member.

Experience with working with people with disabilities and/or personal experience with a disability.

Knowledge of Medicaid, Family Health Plus, Child Health Plus

Ability to effectively work with variety of diverse individuals and effectively manage multiple tasks.

Up beat personality and the ability to effectively convey trust and understanding.

Excellent written and oral communication skills.


RESPONSIBILITIES
Must know, understand, and promote the Independent Living Philosophy.


Assists new consumers in the intake process which includes but is not limited to providing information about Center services, necessary assistance in completion of required forms and provides accurate, up to date information in areas such as, but not limited to disability related laws, self-determination skills, benefit entitlement options, and the independent living philosophy.


Responds to requests for information and referrals from people with disabilities, their friends and family, and other community members in disability related areas by providing accurate information, making appropriate referrals to other Center staff, and by making appropriate referrals to other community agencies and organizations within five business days.

Work collaboratively with other ACES Employment Specialists to identify program participants.

Effectively collaborates with mental health practitioners, VR Counselors, family members, and other members of the multidisciplinary team

Develops effective relationships with area employers in order to facilitate employment of job seekers with disabilities.


Conduct outreach to, and coordinate with appropriate community service providers to foster linkages between these organizations and job seekers with disabilities.

Identify participant's needs and research career options and local resources to address those needs.

Utilizes good consumer interview skills in order to learn about individual preferences, past experiences, hope and concerns regarding employment.

Assist participants in understanding the current job market.

Effectively assists job seekers with disabilities in finding, securing, and maintaining competitive employment.

Assists with finding and accessing resources such as work incentives planning and available transportation.

Provides guidance and assistance regarding benefits and/or entitlements.


Provide individually tailored supports that may include, but are not limited to: on the job mentoring/training, work readiness skills, assistance with interacting with employers, follow-up/follow-along support, and other relevant supports needed to assist participants in maintaining a job placement.

Educate potential enrollees about the NYHBE and the types of insurance programs offered.

Facilitate selection of a Qualified Health Plan in the NYHBE, or when appropriate an, Insurance Affordability Program.

Maintain an expertise in eligibility, enrollment and program specification for various insurance programs available through the NYHBE.


Provide information in a fair and impartial manner which is culturally and linguistically appropriate, and disability accessible, for populations served under the exchange, including those with limited English proficiency.

Assist potential enrollees with grievances, complaints or questions regarding their health coverage or a determination related to their coverage


Keeps accurate written records and statistics of all services provided and provides timely written documentations that meet requirements of funding sources.


Pay:
$34, $36,500.00 per year
