Qualifications

Required:


  • Associate degree or higher
  • RN (3+ years)
  • Full proficiency in speaking, reading, and writing in Spanish
  • 3+ years of experience in ANY of the following:
  • Geriatric Care
  • Senior Care
  • ALL of the following valid licenses/certifications:
  • Valid driver's license
  • Personal transportation
  • Good driving record
  • Auto insurance
  • American Heart Association Basic Life Support (AHA BLS)
  • Registered Nurse (RN) license in California (CA)

Preferred:

Overview
Join our Award Winning Client's team as an Enrollment Registered Nurse for the PACE Program in Los Angeles. Our client has won many awards and operates nationally.

This individual will play a vital role in bringing the PACE (Program of All-Inclusive Care for the Elderly) program to serve frail elders and disabled older adults in underserved communities across Los Angeles.

The Enrollment Registered Nurse serves as a liaison between the Interdisciplinary Team (IDT) and the Outreach Team to ensure a smooth enrollment process for prospective participants.


Key Responsibilities

  • Conduct home visits to evaluate prospective participants for potential eligibility for nursing home level of care
  • Participate in Enrollment Eligibility meetings to review Level of Care recommendations
  • Conduct home safety evaluations for prospective participants
  • Complete Nursing Facility Level of Care (NFLOC) assessments and submit to DHCS for review; update/revise assessment as requested by DHCS
  • Maintain accurate documentation and record all evaluations in CRM software
  • Participate in all PACE Audits by DHCS and CMS
  • Support the Center Director in developing participant retention strategies
  • Collaborate with the Outreach Specialist and IDT for successful enrollment
  • Maintain knowledge and expertise in eligibility, enrollment, and program specifications for the PACE Program
  • Assist with outreach programs as needed to support business development and census growth

Schedule and Shift Details

  • Monday to Friday, 8 a.m.5 p.m.
  • Mostly infield home appointments with occasional onsite work in Los Angeles clinic up to 5 days per month

  • 95% travel to patient homes
