Responsibilities:

Responsibilities for Release Planner:

Conduct rounds with physicians to meet with patients to discuss placement orders
Assist patients with a specific plan for medication management
Include patient in discharge planning
Assist patients with the identification of a support network, instructions for future care and followup
Coordinate care with community agencies, shelters, employment agencies, and drug and alcohol counselors

Qualifications:

Qualifications for Release Planner:

Bachelor's Degree in a human service field
Minimum of 1year experience in a clinical setting, with correctional healthcare experience preferred
Knowledge and experience in the development of discharge plans and coordination with community service providers
Valid CPR card
