Medical Social Worker to do visits throughout
San Diego County.

General Responsibilities:
- Performs initial psychosocial and bereavement assessments and assists in the development and implementation of goaldirected Interdisciplinary Team care planning.
- Conducts ongoing reassessments of patient/family/loved one needs and counseling as required.
- Documents comprehensive psychosocial assessment clearly and concisely in a timely manner.
- Observes, assesses, and brings information to Interdisciplinary Team conferences regarding psychosocial, emotional, physical, and financial conditions affecting the patient and family.
- Assumes the active role of advocacy for the patient/family/support unit.
- Ensures endoflife arrangements are in place and assists with these plans as needed.
- Works with patient and support network to find solutions to psychosocial, financial, legal, and environmental concerns.
- Provides short term crisis intervention and individual or family counseling when indicated. Develops and maintains contact with appropriate community agencies and services in order to promote interagency cooperation and to facilitate related referrals.
- Adhere to all Hospice policies.

- Work independently
- Make accurate and, when needed, quick judgements
- Respond appropriately to crisis outside of a hospital setting
- Acceptance of and adaptability to different social, racial, cultural, and religious backgrounds, affiliations, and practices
Requirements:
- An American Veterans Hospice social worker must have an MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE), and one year of experience in a health care setting.
- Valid driver's license, auto insurance, reliable transportation and willingness to drive to patients place of residence throughout San Diego County.
