The Transition Coordinator is an experienced nurse who is able to provide 1:1 instruction and education to patients/families who will and have experience a surgical procedure in their area of focus (ex. cardiothoracic, orthopedic).She/he will serve as patient advocate, discharge nurse and provide follow up calls to assess and assist the patient's transition home.

Required Qualifications:
Associate's Degree Nursing degree/diploma upon hire.
Washington Registered Nurse License upon hire.

Preferred Qualifications:
Bachelor's Degree Nursing or higher within 3 years of hire.
National Certification in area of specialty.
1 year of experience in care management or utilization review in any setting or successful completion of TIP program or Case Management Orientation Program.
