Job summary
The post-acute care manager provides care to patients who are discharged to post-acute facilities and maintains a caseload according to productivity standards. This role performs and coordinates work with the patient, family or support members, inpatient case management team, and interdisciplinary care team members during post-acute admission for appropriate utilization of services length of stay and safe discharge plan.Essential job functions include running daily reports transitions of care making assignments reviewing goals of care communicating with skilled nursing facilities evaluating effectiveness of care plan conducting comprehensive assessments completing required documentation performing patient outreach coordinating referrals transitions of care providing patient family caregiver education referring next level discharge planning meeting with high volume facilities on overall performance quality readmissions assisting with standard operating procedures policy development as needed.
