Job summary
The Clinical Case Manager facilitates an assigned group of patients through the health care system with the primary focus of efficient, effective, compassionate inpatient care and a safe transition to the outpatient setting.Minimum Education Required
Bachelor's Level Degree or equivalent combination of education and experience.
Required Qualifications
Bachelor's degree, Masters preferred. RN licensure required; five years recent clinical experience applicable to the population preferred; knowledge of clinical practice guidelines, computer data bases, and payer protocols desired.
