Description
Licensure and Certifications Reimbursement~ Experience:
Minimum of three years of Acute Care RN experience within a hospital setting required.
~ Minimum 2 Years of RN Case Management within a Large Acute Care "Hospital".
~Working closely with the collaborative care team, the case manager assesses plans, intervenes, and reassesses an individual's needs to implement a plan of care to meet the needs of that individual
The case manager assesses patient and family for clinical appropriateness for the setting and establishes plans for effective allocation of resources and management of the continuum of care.
Utilization review is conducted according to ShorePoint Health guidelinesCoordinates the overall interdisciplinary plan of care for a patient, from admission to discharge
Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively.
Day Shift Available (8X5's varying days)
Ability to collect, organize, and analyze data. Innovative and creative approach to work. Computer literacy. Minimum five years of clinical e experience in an acute care setting.
Associates or better in Nursing
Bachelors or better in Nursing or related field
