As a Case Manager specializing in substance abuse and mental health at Pristine Recovery, you will play a pivotal role in facilitating the recovery journey of our clients.

Drawing upon your expertise in counseling and case management, you will provide personalized support and advocacy to empower clients to overcome barriers and achieve their treatment goals.

Key Responsibilities:

• Conduct comprehensive biopsychosocial assessments to identify clients' strengths, needs, and treatment preferences.
• Develop individualized treatment plans in collaboration with clients, incorporating input from the treatment team and addressing substance abuse, mental health, and psychosocial factors.
• Provide ongoing case management services, including referrals to community resources, coordination of care with external providers, and assistance with navigating healthcare and social service systems.
• Advocate for clients' rights and needs within the treatment center and in interactions with external agencies, such as insurance companies, legal entities, and housing authorities.
• Monitor clients' progress in treatment, conduct regular checkins, and adjust treatment plans as needed to ensure optimal outcomes.
• Facilitate group workshops and psychoeducation sessions on topics related to substance abuse recovery, mental health wellness, relapse prevention, and life skills development.
• Document all case management activities accurately and in a timely manner, maintaining compliance with confidentiality and regulatory requirements.
• Collaborate closely with the interdisciplinary treatment team, including therapists, psychiatrists, nurses, and peer support specialists, to provide coordinated and holistic care.
• Participate in team meetings, case conferences, and supervision sessions to review client progress, address challenges, and coordinate care plans.
• Stay abreast of current trends, research, and best practices in the fields of substance abuse treatment, mental health counseling, and case management.

Qualifications:

• Master's degree in Social Work, Counseling, Psychology, or related field.
• Current licensure as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC) in the state of [Insert State].
• Minimum of two years of experience in substance abuse treatment, mental health counseling, or related field.
