Role Description:
Oak Street Health provides high-quality primary care to Medicare patients at local centers and also in their homes when necessary.

The Medical Assistant/CIS for House Calls plays a vital role in supporting logistics, clinical care, and documentation in our innovative home-based care program.

The Medical Assistant/CIS for Home Visits will collaborate closely with a House Calls nurse practitioner on a daily basis, working alongside center-based care teams to address care gaps when patients come back from the hospital or face other challenges.

Core functions:
  • Planning visit schedules, routes, and logistics for all visits
  • Assisting nurse practitioners during house calls
  • Recording vital signs, conducting medication reconciliation, providing patient and family education
  • Documenting House Call notes, handling referrals, orders, and follow-ups
  • Managing supplies and equipment
  • Other assigned duties

Requirements:
  • Minimum one year of experience as a Medical Assistant + MA certification
  • Successful completion of all clinical competencies, phlebotomy skills a bonus
  • Daily access to a vehicle with valid license and insurance
  • Ability to travel throughout Chicagoland
  • Proficiency in English and Spanish is advantageous
  • Valid US work authorization
