Our technology-enabled practice (launched 2018) offers multiple specialty service lines for patients across the United States and is powered by our proprietary digital health platform which helps us render the highest quality of care for the following services:

  • In-person encounters (clinics)
  • Inperson encounters (facility rounding)
  • Telemedicine encounters (patients)
  • Peertopeer consults (between PCPs and specialty physicians)

Job Summary:


We are seeking a highly motivated advanced practice provider (physician's assistant or nurse practitioner) to help us support healthcare facilities in the greater Phoenix area with in-person facility rounding and facility telemedicine.

Part-time and fulltime work are available.


What you will be doing:


  • Provide comprehensive Infectious Diseases consultation services at healthcare facilities (SNF, Rehabs, Community Hospitals)
  • Work with inpatient care coordination to assure appropriate discharge plans are established, and if a patient is on IV antibiotic therapy, obtain appropriate followups and monitor.
  • Perform clinical assessment, care recommendations, appropriate antibiotic drug monitoring and medication changes.
  • Provide guidelinebased specialty care with support from our infectious disease specialists.
  • Followup on critical and time sensitive laboratory results and outpatient requests.
  • Establish continuity of care for discharged infectious disease patients by organizing outpatient telemedicine patient follow up.
  • Network and communicate with skilled nursing facilities and other subacute care facilities.
Facilitate access to specialty care between nursing facilities and nodSpecialists infectious disease telemedicine clinics.

  • Demonstrate proficiency in the management and diagnostics of common outpatient and inpatient general infectious disease problems, with support of our infectious disease physicians.
  • Maintain updated consult/patient log while providing services with nodSpecialists or any of the organizations working in partnership with nodSpecialists.
  • Communicate in a clear and concise manner for all nodSpecialists staff needs.
  • Maintain appropriate and timely patient encounter documentation.
  • Maintain all hospital/facility credentialing (if applicable), DEA and state licensure requirements to practice uninterruptedly while providing services with nodSpecialists.
