    Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

    JOB DETAILS AND QUALIFICATIONS


    Are you a motivated and experienced mental health professional? Clackamas Health Centers - Behavioral Health Clinics is seeking a dedicated individual to join our Patient Centered Medical Home team as a Behavioral Health Consultant (classified as a Mental Health Specialist 1 or Mental Health Specialist 2).

    This position offers the opportunity to provide advanced level professional treatment services to primary care clinic patients at both the Beavercreek Health Center in Oregon City and the Sunnyside Health & Wellness Center in Clackamas, OR.

    This is a great opportunity to collaborate with primary care physicians to ensure the best care for every patient


    Primary responsibility for this position will be to work with and provide targeted behavioral and mental health support to a team of primary care providers, including PCPs and CMAs.

    This includes screening patients for appropriate services, crisis needs, PCP requested services, pilot projects, coordination of care, and patient referrals to ongoing mental health services as needed.


    Successful candidates should have the ability to adopt an integrated system of care philosophy, demonstrate understanding of self-determination and patient-centered planning techniques, possess thorough knowledge of mental health, legal, medical, and social resources in the community, and have proficiency in methods and techniques of case management and care coordination related to patients who need help with behavior change related to their medical condition concerns.

    Candidates will also be able to manage a large number of patient contacts during a clinic day, including scheduled appointments and immediate or walk-in referrals from providers, including risk management.

    The ideal candidate will not only have post-graduate mental health experience as a licensed clinician, but addictions experience as well.

    • This position is currently open for a licensed, Mental Health Specialist. Clackamas County will consider unlicensed candidates who have a background providing mental health treatment to individuals in a health clinic or relatable environment serving the vulnerable population. Licensure is however required within three (3) years of hire for applicants who are unlicensed and they must still meet QMHP criteria (outlined below).

    Required Minimum Qualifications/ Transferrable Skills:

    • Qualified Mental Health Professional (QMHP) as established by the Mental Health and Addiction Counseling Board of Oregon (MHACBO)
    A "Qualified Mental Health Professional" (QMHP) means a Licensed Medical Provider or any other person meeting one or more of the following minimum qualifications as authorized by the Local Mental Health Authority or designee:


    • Graduate degree in psychology;
    • Graduate degree in social work;
    • Graduate degree in recreational, art, or music therapy; or
    • Graduate degree in a behavioral science field.


    QMHPs must demonstrate the ability to conduct an assessment, including identifying precipitating events, gathering histories of mental and physical health, alcohol and other drug use, past mental health services and criminal justice contacts, assessing family, cultural, social and work relationships, and conducting a mental status examination, complete a five axis DSM diagnosis, write and supervise the implementation of a ISSP and provide individual, family or group therapy within the scope of their training.


    • If Licensed, must be credentialed as a Clinical Social Worker (LCSW) - preferred, Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT) or Licensed Psychologist (PsyD/PHD) AND a minimum of two (2) years of related experience that would provide the required knowledge and skills to perform the responsibilities of this position.
    • If unlicensed, must have minimum of one (1) year of relevant experience that would provide the required knowledge and skills to perform the duties of the role AND must also receive licensure as a Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT) or Licensed Psychologist (PsyD/PHD) within three (3) years of hire.
    • Must possess and maintain a Basic Life Support (BLS) certificate
    • Must successfully pass the credentialing process

    Preferred Special Qualifications/ Transferrable Skills:

    • Current Licensure (LCSW, LPC or LMFT preferred)
    • Certification as a Certified Alcohol and Drug Counselor (CADC) I, II, III

    TYPICAL TASKS

    • Conducts interviews with patients and collects critical information necessary to complete comprehensive assessments, for both mental health and substance abuse disorders that include psychosocial, developmental, financial and physical factors, substance abuse history, trauma history, diagnosis and mental status; evaluates functional impairments of consumers. Completes the client level of care tool as part of the evaluation process.
    • Develops and implements treatment plans; determines appropriate treatment, skills training and care service needs; matches patient needs with potential services and resources. Refers patients to other internal/external providers to match service needs.
    • Monitors patient progress during treatment. Writes treatment progress notes and maintains electronic and paper case records in accordance with OARs and agency procedures; completes program related assignments as required.
    • Coordinates with external resources, internal and external health care providers and social service personnel to obtain services for patients or to participate in problem solving. Meet with community agencies as needed to coordinate care and meet client needs. Outreaches to resources in the community to engage staff and develop referral sources.
    • Coordinates and consults on the patient's behalf with family, providers, public and private agencies, law enforcement agencies, justice system, schools and others to maximize treatment goals; responds to emergencies and negotiates resolution of problems.
    • Provides targeted interventions and screenings to patients and significant family members in accordance with the treatment plan to meet the treatment needs of the client.
    • Works closely with other agency providers, such as physicians, psychiatrists, therapists, and case managers to evaluate, treat, plan with clients for appropriate care.

    REQUIRED KNOWLEDGE AND SKILLS

    • Thorough knowledge of accepted principles and theories of human behavior, personality and development, and social interaction and development.
    • Knowledge of methods and techniques of Evidenced Based Practices used for therapy for dealing with substance abuse and mental health problems.
    • Knowledge of rules, laws, regulations, and ethics governing the treatment of Mental Health and Substance Abuse consumers and their families.
    • Knowledge of principles and techniques of interviewing, counseling and crisis intervention.
    • Skill and ability to diagnose mental health and substance abuse disorders across the life span.
    • Ability to develop and implement consumer treatment care plans and monitor consumer progress toward established treatment goals.
    • Knowledge of agency services and criteria for eligibility. Knowledge of mental health, substance abuse, legal, medical and social resources in the community.
    • Ability to communicate clearly and effectively both verbally and in written form.
    • Ability to work effectively with other behavioral health professionals and as a member of a professional team.
    • Knowledge of applicable OARs governing service provisions to consumers. Knowledge and ability to document according to Medicaid Standards. Knowledge of standard clinical documentation procedures and requirements. Ability to follow agency specific record keeping procedures, including ability to work within an electronic clinical documentation system.
    • Knowledge of mandatory reporting requirements. Knowledge of legal process regarding voluntary and involuntary treatment/commitment. Ability to be a mental health designee.

    WORK SCHEDULE


    This position is included in the County's alternate workweek program, working 40 hours during a standard workweek of Monday through Thursday (Fridays off).


    Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.


    This position is eligible for hybrid on-site/telework subject to the Clackamas County Teleworking Policy and based on the Department's business needs.

    The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregon or Washington. Washington residents must perform 50% of their time on-site at Clackamas County.
