**CCBHC Care Coordinator

  • Targeted Case Management**REPORTS TO: CCBHC PROGRAM COORDINATOR

We believe that in-person operations are the best way to serve our clients, both directly and through essential operational support.

This position requires in-person attendance with a team at 1 Brooklyn clinic location with some travel in support of the CCBHC Project.


SUMMARY:

Experienced care coordinator needed for new CCBHC program dedicated to integrating mental health, substance use and medical care.

Program will provide care coordination, mobile crisis intervention and ongoing treatment in mental health, medical health and substance use in the Coney Island community.

The care coordinator must conduct phone calls for client's needs, including but not limited to, with mental health, substance abuse and medical intakes and appointments; benefits; housing; community referrals; and social service needs.

The person must facilitate case management sessions, organize priorities of needs and engage the client with establishing goals. Requirements include attending supervision and training sessions and completing documentation of case management services and activities.


JOB DESCRIPTION:


  • Enroll clients in Care Coordination
  • Assist clients with engaging in mental health, chemical dependency and medical services
  • Help clients to navigate care systems
  • Assist clients with health insurance enrollment
  • Assist clients with obtaining benefits
  • Help clients to develop and pursue life goals
  • Develop and implement care plans with clients
  • Complete all case notes and assessments in a coherent and timely manner
  • Ensure that all identified client needs are addressed
  • Identify necessary resources to address client needs
  • Keep client insurance, demographic and contact information current in the medical record
  • Develop, implement and document discharge planning and termination.
  • Work collaboratively with the CCBHC Interdisciplinary Team as well as with outside agencies.
  • Attend all staff meeting and complete all CCBHC and organizational trainings.

EXPERIENCE:


  • 25 years in case management or care coordination preferred
  • Experience with adults with mental health and/or substance abuse needs
  • Excellent written and verbal communication
  • Computer proficient
  • Well organized with strong time management skills
  • Basic knowledge regarding entitlements and other concrete benefits
  • Enthusiasm and selfmotivation regarding identifying client needs and obtaining related resources.
  • Able to work independently and as a member of a team
  • Excellent interpersonal skills, including being able to maintain a professional temperament with both clients and colleagues
  • Working knowledge of computer software and electronic health record documentation.
  • Demonstrated competency in written and verbal skills
  • Able to meet program standards in documenting in client records
  • Ability to work with challenging clients
  • Comfortable with community outreach and home visits
  • Comfortable and competency working with racially, culturally, religiously and ethnically, linguistically, differently abled diverse communities, including but not limited to BIPOC and LGBTQ+ communities.
  • Bilingual skill in a language other than English valued.

SALARY RANGE:
$60_65K Based on experience and licensure/certification.
