- Upholds organizational Policies and Procedures, Principles of Responsibilities, and applicable state, federal, and local laws.

- Provides therapeutic patient care according to physician's orders and a registered nurse's guidance within the scope of practice.
- Transcribes physicians' orders as per policy.
- Communicates directly with registered nurses to implement changes in the plan of care; contributes to the development of nursing care plans (Interdisciplinary Care Plans).
- Administers specified medication orally, hypodermically, intramuscularly, and topically.
- Performs treatments such as urinary catheterizations, bladder irrigations and installations, suctioning procedures, throat irrigations, and enemas; utilizes equipment as necessary; offers nursing care for patients in isolation.
- Carries out patient care functions like taking temperature, pulse, respiration, and blood pressure readings; applying and changing dressings, and bathing; may educate patients on self-administered treatments or procedures; obtains specimens; ensures correct culture collection.
- Assists physicians or other medical staff with examinations and treatments by selecting and passing instruments, providing support and restraint to patients, draping, and other necessary assistance.
- Evaluates patients' conditions within scope by observing signs, symptoms, and reactions to therapy, identifying unexpected and/or significant findings, initiating appropriate emergency procedures when needed, and reporting unusual conditions to the registered nurse promptly as per policy and procedure.
- Identifies barriers to learning. Provides uncomplicated teaching following the registered nurses' education plan.
- Records patient signs, symptoms, observations, and treatments in the medical record.
- Ensures proper functioning of patient care equipment through visual inspection, routine testing, and other methods. Identifies and reports environmental safety concerns to the RN case manager.
- Utilizes effective verbal and non-verbal communication.
- Assists in other assigned areas or units as required by nursing administration to maintain the established standards of care and safety.
- Participates in staff meetings and in-service programs.
- Assists in ordering and distributing unit supplies and equipment.
- Accepts verbal and telephone orders from physicians.
- Administers pulse oximetry, humidification therapy, spirometry, oxygen therapy via mask or nasal cannula, oral, nasal and trach suctioning, and sputum induction.
- Documents care in the medical record.
- Performs other related duties as needed and assists in coordinating the discharge process.
- Required to drive/travel to visit patients' homes.
- Promotes, ensures, and enhances customer service to internal/external customers by demonstrating skills aligned with the organization's philosophy of providing exceptional customer relations and quality service. Age-related competency skills assessments and validations are employed to ensure that employees providing patient care to specific age groups (i.e., infants, adolescents, elderly, etc.) are proficient in recognizing the special needs and behaviors associated with a particular patient population.
- One (1) year of recent (within the last three (3) years) experience as an LVN in a home health/hospice or acute care setting.
