    Pinnacle Home Care Team - Physical Therapist Opportunity in Duval County

    Role Summary:
    As a Physical Therapist at Pinnacle Home Care, you will play a vital role in our healthcare team, delivering Physical Therapy services in compliance with all relevant regulations. You will collaborate with physicians and nursing supervisors to provide skilled care to our clients in their homes.

    Key Responsibilities:
    - Conduct thorough assessments, including OASIS evaluations.
    - Develop and adjust treatment plans with physician approval as needed.
    - Contribute to the creation and implementation of comprehensive client care plans.
    - Educate and guide clients and their families on physical therapy routines.
    - Supervise Physical Therapist Assistants as per guidelines.
    - Maintain effective communication with physicians and team members regarding client care.
    - Promptly and accurately document clinical and progress notes.

    Qualifications:
    - Graduate from an accredited Physical Therapy program.
    - Possess a valid Florida PT license.
    - Current BLS Certification.
    - Hold a valid driver's license with access to a reliable and insured vehicle.
    - Ideally, have at least one year of experience as a Physical Therapist, with home healthcare background being advantageous.
    - Proficiency in electronic medical records is a plus.
    - Ability to work independently with strong observational, judgment, and communication skills.
