    PACE Place's top priority is to assist elders in preserving their independence and offering the necessary support to age in place. Our team recognizes the challenges of caregiving and assists in coordinating all care and services, including access to a clinic, a well-equipped adult day health center, and transportation services. Caregivers can have peace of mind knowing that PACE Place is dedicated to looking after their loved ones.

    Job Summary:

    Evaluate participants, administer physical therapy treatments, monitor progress, provide instructions to participants and/or their families as part of the PACE Place team. Supervise the work of aides, assistants, and students. Carry out assessments, develop care plans, coordinate 24-hour care delivery, keep the interdisciplinary team informed about participants' medical, functional, and psychosocial status, and document changes in participants' medical records. Work hours are typically Monday to Friday at the Center with occasional weekend visits or home appointments with no on-call requirements.

    Qualifications and Requirements:

    Education

    • Hold a minimum of a master's degree in physical therapy from an accredited university, except for those who graduated before 1/1/2003 and possess at least a bachelor's degree in physical therapy or a bachelor's degree along with physical therapy certification.
    • Must be licensed as a Physical Therapist in Florida.
    • Hold a current driver's license and provide proof of auto insurance.

    Experience

    • At least 1 year of experience working with frail or elderly individuals is preferable.

    Skills and Knowledge:

    • Demonstrate proficiency in modalities like exercise, mobilization, manipulation, electrical stimulation, massage, hydrotherapy, and hot/cold therapies.
    • Experience in restorative and rehabilitation techniques to enhance or maintain function, reduce pain, and promote independence.
    • Up-to-date knowledge of geriatric issues and best practices.
    • Strong problem-solving, judgment, and cognitive abilities.
    • Ability to lead and collaborate within an interdisciplinary environment.
    • Passion for caring for the geriatric population.
    • Familiarity with common home safety hazards and methods to create a safer environment.
    • Capable of providing training in injury prevention techniques and functional self-care activities.
    • Must possess or attain First Aid and CPR certification (from an accredited provider) for this role due to participant interaction.
