The LA Conservation Corps (the Corps) is a private, non-profit organization that has been helping at-risk young people develop themselves since 1986.

To date, more than 20,000 young people have participated in the Corps classes and service projects, learning new skills while contributing to the quality of life for all of us in the greater Los Angeles area.


Mission

The primary mission of the LA Conservation Corps is to provide at-risk young adults and school-aged youth with opportunities for success through job skills training, education and work experience with an emphasis on conservation and service projects that benefit the community.


Position Summary


The Case Manager provides comprehensive and integrated case management and support services to Corpsmembers based on a professional assessment of each Corpsmember's needs and the Corps' connections to internal and community resources.

The goal of the Case Manager's work is to provide Corpsmembers with direct social services to help advance their personal and professional skills.

The Case Manager guides Corpsmembers through the Corps' Green Career Pathways program.


Essential Functions

  • Provides integrated case management services, including Corpsmember/family needs assessments and referrals to local resources. Referrals to local community services include medical care, mental health services, childcare, housing, substance abuse prevention, education services, financial literacy, legal services, and community engagement activities. Corps Navigators also make direct referrals to the Corpsmember Development (CMD) team for support with vocational training, postsecondary enrollment, job readiness, job placement, and financial awards.
  • Works with a caseload of approximately 5070 Corpsmembers at multiple program sites, some assigned/attached to CMD contracts/grants; assists with information related to Corpsmembers enrolled in contractrelated followup services, as appropriate.
  • Develops individual service plans with Corpsmembers that are reviewed and updated monthly; tracks progress in meeting Corpsmembers' personal, professional, and educational goals.
  • Meets directly with Corpsmembers to review support service requests and communicate approval or denial of services based on management's feedback; tracks Corpsmembers' supportive service expenses including backup documentation.
  • Coordinates and/or facilitates service referrals to local communitybased organizations and government agencies, utilizing established network and adding to the Corps' list of referral agencies, as appropriate.
  • Responds to Corpsmembers in crisis and provides referrals for emergency services, as needed.
  • Collaborates with Recruitment and Intake staff to meet program enrollment and service delivery goals through frequent contact with Corpsmembers and coordinating (home) visits, office appointments and telephone calls to follow through with individual service plans and other services delivery.
  • Collaborates with Sr. Program Manager of CMD to refer clients to the Corps' Legal Services Program.
  • Tracks and documents contractrelated data for Corpsmembers on caseload (e.g., quarterly Job Readiness Assessments, 300 hours of work experience, etc.) and reports to Sr. Program Manager on a weekly basis via oneoneone (O3) meetings.
  • Maintains and updates files for all Corpsmembers with supporting documentation, while entering and tracking all pertinent Corpsmember data and case notes in the Corps' internal data collection software (Salesforce/ECM) as well as into contractspecific data collection systems.
  • Collaborates with Case & Transition team to refer clients on caseload to life skills, vocational training, and College & Career workshops; identifies Corpsmembers for classes and refers them to the Case & Transition staff for followup.
  • Facilitates/cofacilitates Life Skills, Corpsmember Leadership Councils, and group meetings for Corpsmembers in various settings.
  • Communicates with Work Program and Education staff on a consistent basis to ensure Corpsmembers are successfully participating and progressing through the Young Adult Corps program. Works closely with these departments to ensure staff are informed of Corpsmembers' status and outcomes.
  • Maintains high level of confidentiality and does not use or disclose protected health information about any member or other party in compliance with Corps' policies related to state or federal laws, including the Health Insurance Portability and Accountability Act (HIPAA).
  • Fosters a motivational and supportive environment for Corpsmembers.
  • Coordinates program elements with other Corps staff to achieve program goals and outcomes.
  • Tracks and complies with all due dates, paperwork and other assigned contract requirements.
