    7298 Specialty Homecare-SHC

    Role overview:

    - Provides skilled nursing care in a home setting based on an individual's care plan.

    Requirements:
    - Must hold a CA LVN license and BLS certification

    Experience:
    - Minimum one year of LVN experience required, exceptions possible for those with less experience.

    Job Responsibilities:

    - Contributes to the development of a comprehensive care plan, including goal setting and evaluation.
    - Encourages involvement of staff and family in home care planning.
    - Implements physician's orders within the scope of practice.
    - Evaluates patient's nutritional status and safety concerns.
    - Ensures safe nursing practices and proper medication administration.
    - Maintains accurate patient documentation.
    - Responds efficiently to emergencies.

    Additional responsibilities:
    - Provides comforting nursing measures.
    - Demonstrates cultural sensitivity and effective communication with patients and families.
    - Performs tasks assigned within the unit.

    Restrictions:
    - LVNs in the home setting have limitations on procedures like drawing blood or administering specific IV treatments.

    Pay Range
    $25.00 per hour
