Responsibilities:

Stella Maris is looking for a Social Worker to join our Inpatient Hospice team. The main function of this position is provide social work services to our Inpatient Hospice patients and their families.

JOB RESPONSIBILITIES:

Services include admission, psychosocial assessment, plan of care, linkage with community resources, counseling, crisis intervention, and discharge planning.
Responsible for required documentation in compliance with State and federal regulations.
Assesses bereavement issues of families and makes timely and appropriate referrals to Center for Grief and Loss.
Facilitates effective communication between patient/family and home care team members.
Provides appropriate information and referral services to patients and families/caregivers.
Assumes responsibility for discharge planning as needed
