Description
Make rounds and identifies the problems. Initiates the intervention with appropriate follow up. Performs patient care, assess resident's condition, and takes/monitor residents vital signs. Notifies MD of change in resident's condition and receives and carries out MD orders. Administers medication.- Performs resident assessment as condition warrants.
- Develops, implements, and evaluates the plan of care.
- Initiate the emergency measures according to the facility's policies and nursing practices.
- Communicates with incoming shift regarding resident care needs, and with the unit manager.
- Monitor all the residents to ascertain condition and comfort.
- Follows safety policies in performing resident care.
- Communicates with the physician the status of the resident condition and carry out any orders.
- Communicates with IDT regarding resident care.
- Coordinates with the other discipline on admission of new residents, as well as changes on residents' plan of care.
- Supervise and direct subordinates under him/her.
- Safely administer medication as ordered including but not limited to breathing treatments, parenteral, enteral, Intramuscular, subcutaneous injection, ect.
- Able to assess and provide wound care assessment and treatment.
- Safely practice infection control standards.
- Admits, transfers and discharges residents as ordered by the physician.
- Notify physician and responsible party of resident's for any change of condition.
- Safeguards resident's privacy and dignity.
- Provides assistance to the nursing staff related to care.
- Document assessment care and treatment to the residents.
- Assumes thorough regular observation & evaluation of licensed staff and proficient in the delivery of resident care & are competent to perform all necessary treatment & procedures.
- Must possess a current, unencumbered, active license to practice as a Registered Nurse in this state.
- Must be certified with a current CPR card.
- Responsible for supervision & directing LVN & RN Charge Nurse in providing nursing care.
- Must be able to read, write, speak and understand the English language.
- Must be a supportive team member, contribute to and be an example of team work and team concept.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
- Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
- Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
- Must possess the ability to lift, push or pull 50 lbs. Also, must possess the ability to bend, kneel or stoop.

$ $48.00 per hour

Supplemental schedule:
- Holidays
- Overtime
Weekly schedule:
- 5x Every weekend
- Monday to Friday
- Rotating weekends
- Weekends as needed
Ability to Relocate:
- El Monte, CA 91732: Relocate before starting work (required)
