    Summary of the position: The Older Youth Services Community-Based Case Manager 1 is responsible for providing home-based and community-based quality services for families involved with the Department of Child Services and/or Juvenile Probation through the Older Youth Services contract.

    Description of Duty

    COMPLIANCE

    • Older Youth Services Case Managers will provide community-based and home-based services to clients in compliance with Service Standards and Protocol published by the Indiana Department of Child Services. It is the responsibility of each Older Youth Services Case Manager to know all of the published DCS standards for Older Youth Services that they are providing.
    • Adhere to employee handbook and community-based policy manual.
    • Ensure all case records, correspondence, and conversations regarding clients remain confidential as required by HIPAA compliance regulations.
    • Complete releases and GJR intake forms upon initial contact with client/placement/other parties and update ensuring copies are saved in the clinical record.
    • Document youth contacts in the established database and National Youth in Transition Database (NYTD) web portal (if applicable) by the required due date.
    • Follow George Junior Republic reporting procedures, including but not limited to, ensuring the safety of all clients. This includes assessing and taking action when safety and security is jeopardized and reporting potential abuse or neglect to the DCS Hotline as well as completing critical incident reports.
    • Make initial face-to-face contact with youth within 10 days of referral and communicate initial client contact or lack thereof to referral source and supervisor throughout the 10 days.
    • Adhere to the NASW Code of Ethics and GJR in Indiana "Professional Conduct Guidelines."
    • Promotes client rights and follows George Junior Republic's Code of Conduct.
    • Document client contacts through up-to-date case notes that are maintained in Case Management Pro (electronic database), VPN client files and Kid Traks (if applicable). Record all billable/direct and non-billable/indirect time on a daily basis, utilizing the established database while ensuring an accurate timesheet is maintained as outlined in the CMP methods sheet in the Zimbra email briefcase. If your time is not recorded the same day in CMP, it is required within 48 hours.
    • Clock in and out of ADP.
    • Provide monthly Progress Reports by due date to supervisor to referring agencies and Kid Traks (if applicable) and NYTD web portal as well as any other specific written or verbal information requested by referring agencies.
    • Administer OYS assessments to youth within 30 days of referral. Provide results of assessment to referring agency and NYTD web portal within 10 days of completion. Complete updated OYS assessments annually.
    • Maintain client files ensuring all required documents are in place adhering to the GJR in Indiana Older Youth Services file checklist and OYS Service Standard. Submit and correct file audit identified issues within 14 days of receiving feedback from audits.
    • Document correct use of NYTD activities in notes and reports approved by the State of Indiana in the matrix guide of the Older Youth Services Standard.
    • Submit Attendance sheets for clients on caseload to supervisor by monthly deadline.
    • Completes no less than 12 hours of annual GJR training ensuring compliance with state required training topics annually and bi-annually keeping an accurate record in Relias (Training) database.
    • Adheres to fiscal protocols including keeping an accurate ledger, submitting receipts, spending within stipend limits, and seeking approval for purchases above standard limits.
    • Assist youth in developing Learning Plan for Successful Adulthood based on results of OYS assessments within 30 days of assessment. Provide a copy to referring agencies and in NYTD web portal within 10 days of completion. Update Learning Plans as needed.
    • Complete required applications with every youth that is eligible including but not limited to 21st Century Scholars, Education and Training Voucher, FAFSA, Youth Advisory Board and free credit report.
    • Oversee completion of IARCA packets upon admission, discharge, and three and six-month follow-up for Budget 1 and 2 clients.

    CLIENT CARE

    • Actively work to ensure youth safety and do no harm in a variety of at risk situations including situational/ environmental in client homes and community.
    • Adhere to the interventions, skills, and models as applicable/ required per DCS service standards and George Junior Republic in Indiana Employee Handbook.
    • Establish the necessary rapport with youth to effectively diffuse and deescalate volatile situations.
    • Connect youth to appropriate community resources.
    • Appear at court hearings, case conferences, CFTM's, or staffing as necessary.
    • Ensure youths' rights are respected by adhering to George Junior Republic in Indiana's Youth Rights.
    • Adhere to George Junior Republic in Indiana's Equal Opportunity Delivery of Services policy.
    • Attend and transport youth to OYS events including but not limited to the area's graduation event, Moving Forward Resource Conference, Better Together events, and holiday party. If youth do not require transportation, attendance is still required.

    WORK HABITS/PRODUCTIVITY

    • Display flexibility and adapt to alternative solutions when barriers or dilemmas are presented.
    • Meet or exceed the established minimum direct client contact time weekly, monthly, and annually.
    • Balance administrative duties with client care time with minimal oversight by supervisor.
    • Meet all required deadlines as determined by George Junior Republic and referring agencies.
    • Work no less than minimum hours per position type seeking out supervisor support when barriers are present.
    • Maintain required caseload as determined by program, need, and availability.
    • Balance administrative duties with direct service with minimal oversight by supervisor.
    • Adhere to all George Junior Republic in Indiana Older Youth Services documentation deadlines as outlined in the Older Youth Services Documentation Procedures.
    • Adhere to all George Junior Republic in Indiana Older Youth Services financial guidelines, documentation, and deadlines (ledger, reimbursements, etc.).

    PROBLEM SOLVING/DECISION MAKING

    • Solve problems quickly while staying within all GJR guidelines and policies ensuring safety at all times.
    • Ensure participation in individual, group, and other forms of supervision no less than the required minimum hours per month and frequency of contact as defined by DCS service standards.
    • Follow the chain of command with problem/grievances unless the situation merits otherwise.
    • Uses community resources as primary means of linking youth to long term supports and modeling sustainability in their local areas.
    • Performs job/solves problems with minimal supervision; independent judgement can be trusted.

    EFFECTIVE COMMUNICATION

    • Develop and maintain positive and professional working relationship with referring agency staff and treatment team.
    • Respond to communication needs with referring agency and treatment team members within 24 business hours.
    • All calls and emails are required to be returned within 24 hours Monday through Friday. Weekend calls and emails are required to be returned on the next business day unless you are on call and calls have to be returned within 1 hour.
    • Weekly emails with referring agency on each case is required. Maintain and document (in CMP) weekly communication with referral agency unless otherwise specified.
    • Ensures all written and verbal communication to referring agencies, team members, community partners, and supervisor is professional and appropriate.

    SUPERVISION

    • Inform supervisor of safety issues or other important situations immediately or within the "Guidelines to Contact Your Supervisor".
    • Adhere to the GJR "Guidelines to Contact Your Supervisor" procedure ensuring prompt communication on all high risk and liability issues for clients and agency personnel.
    • Copy supervisor on email communication to colleagues, community partners, and referral sources. Maintains no less than weekly contact with Supervisor returning all calls or emails within 24 hours and is available for regularly scheduled supervision and field shadowing.
    • Engage in individual and group supervision offering positive, supportive feedback to peers.
    • Accepts constructive criticism appropriately.

    WORK QUALITY

    • Written work requires minimal revisions by supervisor.
    • Finished reports/paperwork are accurate, neat, and address the points required in an effective method.
    • Tailor services to address youth's individual needs, values, and culture as well as accommodate referring agency goals/needs.
    • When completing reports and other written documentation, opinion is supported by facts and safety concerns are expressed with clear examples.
    • Participates in ongoing, annual training relevant to enhancing work quality and applies knowledge into direct care action.

    Working Conditions: Must be able to lift 10 pounds on a regular basis. Must be able to walk, bend, and stoop. This position is responsible for working at multiple locations across Indiana, including homes, schools, probation offices, DCS offices, etc.

    REQUIREMENTS

    Education

    Bachelor's degree in social work, psychology, sociology, or a directly related human service field from an accredited college.

    Case Manager 2 – Exempt: BA/BS in a designated Human Services field (i.e., psych, social work, etc.) or advanced degree and/or various specialized certifications, and at least two years' experience working with the designated population AND/OR BA/BS in a Human Service Field (as we defined – SW, Psych, Crim, etc.), Certified in either Substance Abuse Counseling and Prevention or Case Management and one year experience. This would be considered the learned professional.
