    Assisting Hands of Greater Cleveland (AHGC) is expanding and searching to grow the care management team by hiring a Social Worker / Community Educator for Cuyahoga County.

    If you are keen on leveraging your talents to make a meaningful impact in the NEO healthcare sector, then we want to meet you.

    We are in need of someone who enjoys establishing connections with referrers and families in the community, possibly from a healthcare background as a social worker wanting to broaden their horizons.

    Join us if you have a passion for the elderly and possess exceptional presentation skills, organizational abilities, experience with PowerPoint, and preferably in the senior care industry.

    Qualifications:

    • LMSW, LBSW preferred
    • Experience in senior care industry
    • Occupational Therapist, Physical Therapist, LPN, RN considered
    • Passion for the elderly essential

    Primary Responsibilities:

    • Identify local influence centers for lead sources
    • Build and maintain referrer database
    • Conduct training on dementia care and Parkinson's disease care
    • Perform client assessments and create care plans
    • Arrange and conduct presentations of AHGC services
    • Participate in trade shows, conferences, and networking events
    • Utilize CRM system to track leads and follow-ups
    • Meet weekly visit frequency targets and report activities
