Description

Experienced Case Manager for Middlesex/Somerset Counties.

We seek a self-motivated and compassionate individual with experience working with adults

with intellectual and developmental disabilities.

Qualifications
-
One year of proven experience working with adults with developmental disabilities

Excellent written and verbal communication, reliable car, flexibility to attend meetings when needed during the day
:

The state of New Jersey requires Covid vaccination for this position
:

Must pass criminal background and CARI check
:

A bachelor's degree in psychology, sociology, social work, or related field.

Responsibilities
-
Assessing service needs, developing the New Jersey Individualized Service Plan (NJISP)
for access to services, and liaison with clients, families, programs, and service providers
:

To establish achievable goals, outcomes, and service eligibility
:

-
Development of a Person-Centered Planning Tool (PCPT) is a mandatory discovery tool
used to guide the person-centered planning process and assist in developing the NJISP
:

-
Ongoing monthly monitoring of clients
:

-
Coordinate and facilitate team meetings off-site at either Residential and/or Day
Programs to identify service needs.

The caseload varies from 10 to 30 consumers.

Pay is $1,500 - $4,500 Monthly, commensurate with experience and caseload.

Must reside in the State of New Jersey
