Must have strong analytic skills, interpersonal skills, communication skills, basic computer skills, and assessment skills. Must demonstrate the ability to exercise independent judgment. Needs to be self-motivated as well as have the ability to motivate others. Needs to have a broad understanding of Long Term Care and enjoy working with elders. Needs to have a working knowledge of both state and federal regulations as they pertain to Long Term Care. Working knowledge of the current RAI processes in accordance with the regulations.


MDS coordinator Job Description and Duties:

This is a highly visible position which requires daily interaction with direct care givers, residents and family members as well as with a variety of regulatory agencies.

Duties include but are not limited to the following:

  • Manages the resident assessment process including the completion of the MDS and Care Plan in an accurate and timely manner (per state and federal regulations).
  • Schedules each resident care plan meeting and the notice to all participants of the interdisciplinary team, including the resident and /or the resident representative.
  • Promotes the resident's optimum wellness and rehabilitation potential for each resident
  • Maximizes reimbursement
  • Assures compliance with state and federal regulations
  • Monitors systems for capturing appropriate supporting documentation
  • Acts as a role model for professional nursing
  • New admission and discharge tracking for assessment and payment purposes
  • ICD10 coding as required
  • Managing the setting of assessment reference dates and scheduling as per the RAI manual including OBRA and PPS assessments
  • Case management of Medicare and Medicaid residents, and of Managed care residents as needed
  • Attends compliance education and trainings as required.
  • Actively communicate with Health Concepts consultants for an RAI/PPS issues, concerns, or questions.
  • Attends MDSC meetings as required.
  • Any other duties as assigned by the MDS department head.

Qualifications (Requirements, Education, License, Experience):


  • RN or LPN licensed to practice in RI
  • 1 year of clinical experience in Long Term Care
  • Current RAI assessor certification and/or completed MDS training program
