- Work as an integral part of the Arkos Health Interdisciplinary Team
- Execute and contribute to the patient's care plan; make updates and revisions as necessary
- Assist in organizing comprehensive care, which includes arranging referrals to other interdisciplinary services (Home Health, Palliative Care, Social Services) to prevent the need for further acute care
- Manage caseload based on client needs and/or contractual agreements
- Conduct face-to-face visits at least once every 2 weeks for assigned patients enrolled in Intensive Case Management. Additional in-person or phone visits may be required based on patient needs or contractual obligations
- Perform Clinical and Social Assessments (CSA) to devise a care plan during the initial care visit with the patient
- Communicate with the patient's healthcare providers and specialists regarding the patient's status and any changes in condition
- Engage with Arkos Health leadership and contracted providers to address changes in patient status, concerns, or issues through participation in weekly ICT meetings
- Complete medical reconciliation for all transitional care patients within 30 days of initiating TCM contact
- Current, unrestricted LPN License and graduation from an accredited LPN program
- Current BLS/CPR certification
- Annual maintenance of a negative TB test
- Possession of an Arizona Fingerprint Clearance Card
- Proof of Hep B vaccination/titer (Company provides or may sign a declination form)
- Up-to-date flu shot (or adherence to waiver/mask policy)
- Profound knowledge of healthcare services and medical terminology
- Previous experience in case management is desirable
