The Care Coordinator is responsible for the overall coordination of intensive, family-centered, community-based services. This position facilitates Child and Family Team (CFT) Meetings and guides the evolution of the Plan of Care in order to promote a successful home placement for the referred youth. The Care Coordinator will assist with linkages, resource identification, and other supportive services to effectively promote a successful home placement. The position focuses on accessing and building upon the family's natural strengths and community resources. This position may also be asked to participate in an after-hours on-call rotation.


PROGRAM DESCRIPTION:


The Orange County Wraparound program serves children, youth, and non-minor dependents ages 0-21 who are involved with Orange County Social Services, Orange County Healthcare Agency and/or Orange County Probation.

This program provides a collaborative and highly individualized process for developing specific resources and services for the referred child and their families.

The goal of the program is to utilize a strength-based approach to maximize the capacity of each family to meet the child's needs, maintain the child in a family based-setting, and prevent or reduce the need for Short-Term Residential Therapeutic Program (STRTP) placement.


JOB DUTIES & RESPONSIBILITIES:


  • Effectively manage and coordinate all aspects of the Child and Family Team (CFT).
  • Perform initial and ongoing strengthsbased assessments of children and families. Identify and prioritize family's needs across all domains.
  • Schedule and facilitate all Child and Family Team (CFT) meetings; involve the family in the planning and decisionmaking process.
  • Role model positive communication and conflict resolution skills.
  • Identify opportunities and resources to promote family strengths, selfsufficiency, and independence.
  • Coordinate the development of both Plan of Care and Safety Plan and ensure services are relevant to established goals and needs as expressed by the Child and Family Team (CFT).
  • Maintain proficient and accurate documentation adhering to all program and contract standards for compliance, productivity and timeliness.
  • Interface and collaborate with Orange County Social Services, Orange County Healthcare Agency, Orange County Probation, court systems, law enforcement, schools, health care providers and other community liaisons.

KNOWLEDGE, SKILLS & ABILITIES:


  • Must be proficient in Microsoft Word, Excel and Microsoft Outlook and able to navigate EHR systems.
  • Demonstrate knowledge of crisis management skills and can perform such skills as necessary.
  • Ability to remain organized and manage multiple tasks at once.
  • Able to establish and maintain appropriate rapport families and service providers.
  • Knowledge of child development and/or the child welfare system.
  • Ability to adopt a Team Concept and work effectively as a member of a multidisciplinary team.
  • Experience with leading and facilitating groups and meetings.

PHYSICAL REQUIREMENTS:


  • Ability to drive a motor vehicle without restrictions, often in heavy traffic and frequently long distances (one hour or more) to ensure safe transport to client settings and to safely transport clients to appointments as needed.
  • Senses of vision, hearing and smell sufficient to regularly operate a motor vehicle, to visualize threats in the environment or risks to clients, and to operate safely in the field.
  • Required to occasionally lift and carry 1020 pounds.
  • Regularly required to sit, stand, climb stairs (1 or more flights) and walk while in route to and during family visits.
  • Regularly required to sit and use computer in office settings.

Requirements:


GENERAL REQUIREMENTS:


Education/Experience

  • Bachelor's Degree in social work, psychology, nursing, occupational therapy, or a related field from an accredited college or university.
  • One year experience in human services, preferably case management.
  • One year of experience working with target population.

Licensure, Registration, Certification

  • Valid CA driver's license and proof of automobile insurance.
  • Completion of CPR/First Aid certification within the first thirty (30) days of employment
  • Ability to read, write, speak, translate and perform bilingual duties/responsibilities if a bilingual language is designated.
