Summary/Objective:


The Long-Term Foster Care (LTFC) Social Worker/Case Manager provides social work and case management services to unaccompanied minors in foster care placements through the Office of Refugee Resettlement.

Services provided include, but are not limited to, monitoring, assessing, and providing supports and services to youth and families in the LTFC program to ensure stable placements that meet licensing and other regulatory requirements.

All services are provided and documented in accordance with local, state, and federal requirement


Essential Functions:


  • The LTFC Social Worker/Case Manager will perform duties including, but are not limited to, the following:_
  • Assess the needs of youth in care and develop an Individual Service Plan (ISP), including screening for human trafficking concerns and documenting the provision of services in the youths case file.
  • Provides regular individual meetings with foster children and resource parents as outlined in the agency program statement, including maintaining direct contact with each assigned youth and family.
  • Ensures compliance with case plan, visitation, and coordinate monitored visits when needed and appropriate, including the monitoring of these visits.
  • Provides and coordinates supportive services for youth and resource parents.
  • Identify, contact, and vet potential sponsors for the youth.
  • Completes, reviews, and submits significant incident reports (SIR) as required by agency, CCL, and ORR guidelines and/or other regulatory agencies.
  • Assist in the recruitment, orientation, and evaluation of prospective resource parents, including conducting home studies and providing trainings.
  • Coordinates with the assigned Clinician for the provision of individual and/or group therapy with youth as specified in the ISP.
  • Document service interventions and clinical observations including case notes, initial assessment reports, and quarterly progress reports.
  • Ensures foster resource parents are compliant with agency, CCL, and ORR regulations and requirements.
  • Assists with the annual evaluation of foster parents.
  • Assists with placement referrals and initial placement decisions.
  • Accompany state licensing or other county monitors during inspections in the foster home.
  • Participates in on call procedure for foster parents and intake.
  • Represents the agency in meetings with county placing agencies and other collaborative associations.
  • Attend all staff and unit meetings.
  • Any other duties as requested.

Qualifications

  • Required Education and Experience:_
  • A Masters degree in a behavioral science from an accredited institution of higher learning
  • Must have a licensed vehicle with a minimum insurance required
  • At least two years experience in adoption and/or foster care working with children and families.
  • Bilingual Spanish fluency
  • Preferred Education and Experience:_
  • Licensure as a Clinical Social Worker, Marriage & Family Therapist, or Licensed Professional Counselor.

Pay:
$ $33.80 per hour

Expected hours: 40 per week
