Essential Duties and Responsibilities:

The Best Practices Program is designed to enhance assessment, intervention, and treatment services for individuals experiencing homelessness.

By conducting thorough assessments and coordinating care, the program aims to facilitate easier access to Substance Use Disorder (SUD) treatment within a Housing First approach.

SUD Care Coordinators will dedicate their efforts exclusively to SUD intervention, referrals, and treatment alternatives.

They will facilitate smooth transitions for clients to treatment providers, enabling them to prioritize stabilization and recovery while remaining in the shelter system.


  • Maintain accurate client data in the agency database, ETO, ECM and county database HMIS daily.
  • Conducts individual counseling sessions with each caseload participant.
  • Conduct thorough assessments to understand each client's substance use history, treatment needs, mental health status, social support, and any other relevant factors that may impact their recovery.
  • Proactively connects participants to suitable internal and external resources based on their treatment needs, and follows up on their progress and status.
  • Upholds appropriate and ethical boundaries with participants at all times.
  • Engages in crisis intervention and maintains communication with the treatment team in response to unforeseen situations.
  • Records all services provided and any other necessary documentation in the participant record.
  • Collaborates with the treatment team to develop and assess the effectiveness of individualized treatment plans and monitor participant progress.
  • Ensures timely and accurate completion of all clinical documentation, entering data into various electronic systems as required.
  • Ensures a progress note is made for every scheduled appointment, regardless of whether it is kept, missed, or canceled.
  • Advocate on behalf of the client to ensure they receive the necessary care, support, and resources required for their recovery journey.
  • Transports clients in the agencyprovided vehicle for appointments as required.
  • Actively engages in collaborative efforts with fellow staff members to ensure that clients receive the necessary support they require.
  • Maintain high ethical standards in all aspects of the role. Adhere to professional codes of conduct, respect client confidentiality, and prioritize the wellbeing and rights of clients at all times.
  • Be prepared to respond to crisis situations promptly and effectively, providing immediate assistance and access to appropriate resources when needed.
  • Responsible for promptly notifying the manager of any work orders that need to be submitted or supplies that need to be ordered.
  • Responsible for attending staff meetings, ensuring timely completion of training, and accurately completing timecards.
  • Perform related duties as assigned by the supervisor.
  • Maintain compliance with all company policies and procedures.

Education and/or Work Experience Requirements:

Some examples are:

  • High School Diploma or equivalent: A minimum educational requirement to demonstrate basic literacy and communication skills.
  • Associate degree required.
  • Registration or Certification as a substance abuse counselor through a DHS (Department of Human Services) agency.
  • Employees are required to maintain an active AOD registration or certification throughout their employment.
  • Minimum two years experience in substance use and or behavioral health care treatment
  • Excellent communication skills: Ability to effectively communicate with shelter residents, colleagues, and external stakeholders.
  • Crisis intervention skills: Capacity to handle crisis situations and deescalate conflicts.
  • Empathy and compassion: Demonstrating genuine care and understanding for individuals experiencing homelessness.
  • Ability to work in a team: Collaboration and cooperation with other shelter staff, volunteers, and external agencies.
  • Excellent computer proficiency (MS Office

Required Qualifications:


Employment Eligibility Verification:


  • Must provide documentation to establish identity and employment eligibility in compliance with legal requirements. This may include a U.S. passport, Permanent Resident Card, or other acceptable forms of identification.

Negative Tuberculosis Test:


  • Must provide documentation of a recent negative tuberculosis (TB) test result to ensure a safe and healthy work environment.

Background Screening:


  • Successful completion of a comprehensive background screening, which may include criminal record checks, employment history verification, and reference checks.

Pre-Employment Drug Screening:
