Coordinate the development and maintenance of adequate record keeping systems for client intake.

• Coordinate the development and maintenance of adequate record keeping systems for client management or activities that include involvement with consumers in daily, work or social activities.
• Function as a liaison between PAS and case coordination units and referrals

- interactions with consumers in the implementation of individual service plans, education plans or behavior programs.
- gathering behavior program data by interacting with or observing consumers to determine appropriate program implementation.
- involvement with consumers in programming at residential, work sites and other venues.

• Meet regularly with the developmental instructor in order to coordinate service delivery to clients.
• Participate in conferences, workshops, seminars etc. in order to increase knowledge, skills and abilities in the work field.

The Program Director must have at minimum a bachelors degree in the human services field or in social work (other credentials may apply).

This is a part time position, hours per week. This PTS site is located in the Mount Greenwood neighborhood in Chicago, IL.
