The ideal applicant will be responsible for:

  • Coordinate home care services to clients, ensuring the required level and the number of services are provided timely and safely as prescribed by the client's plan of care/authorization
  • Collaborate with other members of the home care team to select home care workers to ensure that the defined unique needs of the clients can be met.
  • Communicate and record service modifications, patients' status change, service terminations, inability to cover required hours, and other odd events.
  • Provide support and assistance to home care workers in assuring they understand their role and follow the client's plan of care and agency's policies and procedures.
  • Assure that assigned home care workers use the visit verification system appropriately to report and verify timely attendance as well tasks performed for clients during service hours.
  • Manage field employees' schedules to prevent unnecessary overtime
  • Perform assigned rotation on the dashboard system once a week.
  • Supports organization's mission by striving for excellence in all aspects of their job with a focus on positive interpersonal skills with coworkers.
  • Adheres to the organization's policy regarding absenteeism and appearance.
  • Attend weekly meetings with Director of Coordination and provides reports as requested by Supervisor.
  • Escalate any concerns pertaining to client or filed employee to a supervisor.
  • Comply with all computer policies and procedures
  • Accept any other responsibilities that management deems appropriate to the position

  • A positive attitude and ability to work independently with mínimal supervision
  • Excellent interpersonal, team player, and communication (written and verbal) skills
  • Strong multitasking, documentation, and organizational skills
  • Proficiency in computer work, especially the Microsoft office suite
  • Be a minimum of 18 years of age, able to read and write English and to carry out directions
  • Home Care coordination experience preferred
  • Bilingual Preferred
  • Ability to work 8am4pm shift and/or on call position on a rotational basis as needed
  • No travel is necessary as this is an inoffice position
  • OT is only needed in emergency in which not all cases are covered during work hours
