Home Visit rate of $50 per visit after training at hourly rate

Responsibilities

Evaluating the effectiveness of the plan of care for the individual:


  • Assessing the participant's health, welfare and unmet needs:

Developing back-up plans and goals with the client:


  • Communicating with SeniorsPlus Care Coordinators to assist clients in receiving appropriate, effective and efficient services to achieve the maximum amount of independence possible:
-
Positively impacting and improving the well-being and health care outcomes for disabled children, adults and older people who are participating in at-home, long-term care programs as prescribed.

Qualifications
-
Bachelor's degree in Health or Social Services:


  • Familiarity with the long term care system a plus, strong computer and organizational skills required:
-
Valid Driver's License a Must

Job Type:
Part-time


Pay:
From $22.00 per hour
