Position Summary:


Working closely with and reporting to the Youth Connections Project Manager, the Youth Connections Coordinator will implement and ensure follow-through on client family assistance plans.


This position is grant funded and contingent upon the funding associated with the respective grant.

Essential Duties and Responsibilities:


  • Work with project staff and partnering agencies, as applicable, and implement the coordination of recovery and wraparound support services.
  • Coordinates with other agency-based services and community-based agencies as applicable.
  • Coordinate individual, group, and family peer supports utilizing best practices.
  • Maintain an adequate caseload and practice time management skills to assure the necessary level of services is provided to participants and assist with follow-up services.
  • Keep abreast of new trends and advancements in the area of behavioral health and serving youth, young adults and families.
  • Attend organization and departmental staff meetings as required.
  • Coordinate with evaluation team on evaluation requirements.
  • Ensure that provided services adhere to confidentiality and standards for quality of care according to federal and state regulations, and agency policies and procedures, and fidelity.
  • Responsible for maintaining confidentiality, including information covered by HIPAA.
  • Perform all other duties as assigned.

  • Possess excellent oral and written communication skills.

  • Ability to relate well with people and to use good judgment.
  • Experience engaging community members, especially working with diverse Native American Cultures.
  • Must be able to travel to conferences or relevant staff trainings.
Preference in hiring is given to qualified American Indians in accordance with the Indian Preference Act (Title 25, U.S. Code, - 472 & 473).

Education and/or Experience:


  • Must have High School Diploma or Equivalent.

Certificates, Licenses, Registrations:


  • Must attend Mandated Reporter Training or obtain certificate within (3) months of employment.
  • Possess a current, valid California driver's license, driving insurance and a good driving record are required for this position.
  • Must be CPR and First Aid certified or at-least within 3-months of employment.

Contingencies

  • Must be fully vaccinated against COVID19 prior to the first day of work.

Physical Work Environment:

While performing the duties of this position, the Youth Connections Coordinator is situated in a standard office environment within the health center, where there is little or no exposure to variations in the weather or other similar elements.

The Youth Connections Coordinator will move throughout the health center to meet with supervisors and co-workers but maintains limited patient/client contact.


Consequently, the Youth Connections Coordinator must:

  • Possess the mobility to work in a standard office setting (i.e. walk, stand, or sit for extended periods of time);
  • Possess the ability to use standard office equipment to write, type, copy, fax, or perform other duties;
  • Possess the strength to lift and carry supplies weighing up to 15 lbs. repeatedly and up to 45 lbs. intermittently;
  • Possess the visual acuity to read printed material and a computer screen; and
  • Demonstrate hearing and speech capabilities that allow him/her to communicate in person and over the t
