    Job description


    Contracted medical staff will be working alongside uniformed Customs and Border Protection (CBP) Agents/Officers in secured federal facilities along the southwest border of the United States.

    These secured federal facilities are staffed and monitored by CBP personnel and/or contracted security staff 24 hours a day, 7 days a week to assure a safe working environment to include the co-located medical unit.

    Contracted medical staff work together with CBP personnel to provide health care services for persons of all ages, to include infants, children, adults, and pregnant females.


    Mission


    CBPs Office of the Chief Medical Office (OCMO) employs a family medicine model to provide frontline medical care for persons in CBP custody.

    This model provides a team of medical professionals comprised of physicians, advanced practice providers (e.g., physicians assistants, nurse practitioners), clinical support staff (e.g., Licensed Practical/Vocational Nurses, Paramedics, EMTs, Certified Medical/Nursing Assistants), and other support staff (Patient Safety Risk Managers and Electronic Health Records Specialists).

    These contracted medical teams provide basic assessment and medical treatment for persons in CBP custody to include emergency care as needed.

    OCMO is committed to ensuring a system of care that is ethical, responsible, and accountable through surveillance and monitoring activities that are in place.

    DHS/CBP Contracting Officer Representative (COR) and/or Contracting Officer, as applicable



    BASIC REQUIREMENTS


    BASIC REQUIREMENTS

    Licensure/Certification:

    • Candidates must have a full, current, and unrestricted license or registration as a Doctor of Medicine (M.D.) or as a Doctor of Osteopathy (D.O.) valid in the location in which the work is performed. Licensure must be maintained current, and any required education, trainings, continued learning to maintain licensure must be completed within the required timeline for completion by the licensing authority. A temporary license will not be accepted.
    • Candidate must not have had any professional licensure or certification withheld, suspended, revoked, or denied, and the candidate must not have surrendered or allowed a professional license or certification to have expired or lapsed because of an investigation or disciplinary action.
    • Candidate must not have been convicted of, pled guilty to, pled nolo contender to, or received probation before judgement for any crime other than a minor traffic violation, the record of which has not been expunged.
    • Basic Life Support (BLS) Certification must be from one of the following approved

    organizations:
    a. American Heart Association (AHA) BLS for health care providers
    b. American Heart Association Advanced Cardiac Life Support (ACLS), if applicable
    c. American Red Cross CPR/BLS for the professional rescuer
    d. American Safety and Health Institute (ASHI) CPR and AED for healthcare providers and professional responders
    i. Course components must include Adult, Pediatric, and Infant CPR, Foreign Body Airway Obstruction, and Automatic Defibrillator (AED) proficiency
    ii. Applicant/ Employee must be certified upon hire and may also be a
    certified instructor
    iii. BLS must be renewed prior to expiration date, and always kept in an active status

    • Current board certification in Pediatrics by the American Board of Medical Specialties (ABMS) or American Osteopathic Association (AOA) approved board. All other board certifications will be reviewed on a case-by-case basis.

    Req Cont:

    • Subsequent to obtaining a Doctor of Medicine or Doctor of Osteopathic Medicine degree, a candidate must have had at least 1 year of supervised experience providing direct service in a clinical pediatric setting, i.e., a 1-year internship or the first year of a residency program in a hospital or an institution accredited for such training. For purposes of this standard, graduate training programs include only those internship, residency, and fellowship programs that are approved by accrediting bodies recognized within the United States or Canada.
    • Residency trained and board certified is required.

    Education/Training:

    Degree:
    Doctor of Medicine, Doctor of Osteopathic Medicine or equivalent from a school in the United States or Canada.

    The degree must have been accredited by the Council on Medical Education of the American Medical Association, Association of American Medical Colleges, Liaison Committee on Medical Education, Commission on Osteopathic College Accreditation of the American Osteopathic Association, or an accrediting body recognized by the U.S.

    Department of Education at the time the degree was obtained.

    Degree from Foreign Medical School:

    A Doctor of Medicine or equivalent degree from a foreign medical school must provide education and medical knowledge equivalent to accredited schools in the United States.

    Evidence of equivalency to accredited schools in the United States is demonstrated by permanent certification by the Educational Commission for Foreign Medical Graduates, a fifth pathway certificate for Americans who completed premedical education in the United States and graduate education in a foreign country, or successful completion of the U.S.

    Medical Licensing Examination.


    Knowledge, Skills, and Abilities:

    • Expert knowledge of the current standards of practice in clinical pediatric medicine.
    • Ability to proficiently provide supervision, oversight, consultation, coordination, and direction for pediatric medical services, to include emergency care if needed.
    • Ability to provide input for congressional inquiries and to effectively communicate with
    Federal Agencies and other organizations as indicated.

    • High degree of independence, initiative, clinical judgment, and follow-through on a wide
    range of sensitive, complex, and clinical pediatric issues.

    • Teaching experience acknowledging a level of expertise and capacity for developing and
    accessing clinical pediatric educational programs.

    • Knowledge of issues confronting the healthcare delivery system, including specific problems and concerns of special pediatric population groups.
    • Knowledge of program policies and guidelines, and operating procedures relating to healthcare delivery systems.
    • Knowledge of organizations and responsibilities of all levels of government operations.
    • Ability to work in a multi-cultural and multi-lingual environment.
    • Ability to utilize telephonic interpreter/translation services to complete assigned duties if
    not fluent in a language the patient understands.

    • Ability to adapt to sudden changes in schedules and flexibility in work requirements.
    • Ability to communicate proficiently in English (verbal and written) to develop positive rapport with pediatric patients and/or their parents, co-workers and other stakeholders.
    • Ability to establish and maintain positive working relationships in a multidisciplinary environment.
    • Ability to navigate in an electronic work environment including electronic health records,

    • Moderate proficiency in, common Microsoft Office programs, specifically Microsoft Word, Excel, Outlook, and SharePoint.
    • Knowledge of regulations (HIPAA/Privacy Act) regarding the confidentiality of patient
    medical records and information as well as Personally Identifiable Information (PII).

    • Ability to initiate and maintain compliance with DHS and CBP policies and any overriding federal standards/policies, or accreditation standards that may apply.
    • Ability to maintain compliance with standard operating procedures and protocols.
    • Ability to perform record keeping functions in accordance with program policies and the system of records management.
    • Ability to use the electronic medical record and maintain an "active status" for use and
    complete all required trainings.

    • Ability to utilize the electronic medical records system for all documentation, to include, but not limited to all patient care, monitoring, medication administration/observation documentation, and input of any paper records as applicable.
    • Ability to maintain non-involvement in the security aspect of patient care and custody.
    • Attends general staff meetings while on duty, and participates with medical quality management initiatives, medical sentinel event reviews, and other reviews as applicable.
    • Assures a safe and clean working environment and assists in the disposal of biologically
    hazardous waste on a regular basis if performing services in the medical unit.


    • Ability to perform quality control of laboratory reagents and perform CLIA waived
    testing as needed for pediatric patients.

    • Ability to completes all annual training or additional training as directed.
    • Ability to travel to each assigned medical unit at least quarterly.
    • May be required to travel.
    • May be required to serve at various medical units within sector/region or cover rotating
    on-call coverage.

    • Ability to assist with patient care in time of staff shortage or surge.

    Duties Cont

    • Ability to be available telephonically for the assigned medical units during core/non-core
    hours for pediatric needs or pediatric care guidance.

    • Ability to telework may be available on a very limited basis, based upon CBP needs and Contracting Officer Representative (COR) approval.

    Major Duties and Responsibilities:

    • Serves as the on-site regional pediatric medical authority within the scope of services provided by DHS/CBP, providing supervision, oversight, consultation, coordination, and direction for pediatric medical services, to include emergency care if needed. Will be required to work on-site at a medical unit.
    • Provides pediatric medical direction and necessary guidance/oversight as required by state licensure for appropriate advanced practice pediatric provider supervision, oversight, consultation, coordination, and direction for all medical services, to include pediatric mental health in the absence of a behavioral health advisor.
    • Clinical supervisory authority over pediatric mid-level providers, as assigned.
    • Provides on-call pediatric coverage for assigned medical units/sector 24 hours a day/7 days a week, however, call coverage may rotate based upon staffing.
    • Responsible for developing and coordinating all aspects of pediatric healthcare delivery
    at his/her assigned facilities.

    • May provide direct care to patients in accordance with current accepted practice, licensure, certification, credentialing and or granted privileges.
    • Provides age-appropriate screening, treatment, and patient instruction pertinent to the discipline and facility setting for acute and chronic conditions for pediatric patients.
    • Acts as an initial reviewer for all outside pediatric referrals/consults to specialty services, to include hospital or emergency care that exceed services offered at the medical unit. Audits referrals and post-referral follow-up, as needed, as part of the medical quality management program.
    • Provides oversight for all pediatric patients requiring enhanced medical monitoring, and completes rounds as needed.
    • Will be expected to respond to and/or coordinate response to medical emergencies and
    medical sentinel events, as needed.

    • Seeks guidance from/collaboration with the Medical Director, Deputy Medical Director, Behavioral Health Advisor, Physician Supervisor, or other leadership as appropriate.
    • Serves on the national and local medical quality management committees and oversees pediatric focused root cause analyses and quality improvement projects/initiatives in collaboration with the national and local medical quality management committee.
    • Performs record keeping functions in accordance with program policies and position.
    • Maintains functional proficiency and ease of use utilizing electronic health records as
    required by, and appropriate to, position.

    • Completes all initial, annual, and ad hoc training or competencies as required/assigned.

    Responsibilities Cont

    • Maintains patient confidentiality, and confidentiality of medical records, in compliance with the Privacy Act and HIPAA regulations in all work activities.
    • Adheres to, and maintains awareness of DHS and CBP Policies, Procedures, Directives, and Operational Memoranda.
    • Leads, attends, and participates in general/medical staff meetings at the local and national
    level, providing pediatric focused guidance and oversight, as needed.

    • Performs pediatric chart reviews as part of medical quality management, focused professional practice evaluation (FPPE) for pediatric APPs, on-going professional practice evaluation (OPPE) for pediatric APPs, collaborative annual performance evaluation for pediatric APPs, pediatric based medical sentinel event reviews, quality assurance and perform improvement initiatives with pediatric focus. Completes summaries of all completed pediatric chart reviews, identifying trends, performance concerns, best practices.
    • Management and oversight of the Peer review program for pediatric APPs assigned as
    part of OPPE for assigned pediatric APPs.

    • Performs a mid-term review of performance with assigned pediatric APPs as part of an annual performance evaluation process, addressing any clinical practice concerns if identified.
    • Provides emergency care to employees or visitors for life, death, loss of sight/limb situations until Emergency Medical Services Arrives, as needed.
    • Other duties as assigned.

    Supervisory Controls:

    • Works under the administrative direction of the Deputy Medical Director and in his/her absence the Medical Director.
    • Works under the clinical supervision and/or direction of the Deputy Medical Director or
    in his/her absence the Medical Director.

    • In this position, the employee will serve as a contractor to the federal government and does not have any federal authority or rights.

    Physical Demands:

    • Able to walk, stand, bend, or sit for extended periods.
    • Able to reach to retrieve documents or supplies.
    • Able to carry materials and/or file boxes up to 30 pounds on an infrequent basis or assist
    patients into/out of a wheelchair.

    • Must be capable of standing on hard surfaces (cement floors) for long periods of time.
    • Must perform the duties in a stressful and often austere environment without physical limitation.
    • Must be able to respond to a medical emergency and perform BLS/CPR as needed
