LSS of Northern California is a Non-Profit agency providing services in supportive housing and money management. LSS responds when people are vulnerable because of homelessness, mental illness, drug addiction or disaster. Providing support services and compassionate guidance, we help individuals and families to stabilize their lives and become self-sufficient.


Program Description:


Bernal Gateway Apartments is a 54-unit permanent supportive housing program for families located in the Mission District of San Francisco. The program serves primarily low income families dealing with issues such as long term poverty and unemployment, immigration status, and family stability. The community includes school age children as well as Transition Age Youth. The services team is comprised of a full time Program Case Manager, a part time program Activities Coordinator, and support from Family Housing Services Program Manager. The team works together to ensure families are provided with necessary resources to ensure individual and family resilience and housing stability.


735 Davis is a newer 53-unit permanent supportive housing building with a roof top garden that is located in San Francisco on the Embarcadero. The building provides housing for 53 adults with a portion of the apartments (15) being supported by a Local Operating Support Program (LOSP) subsidy for formerly homeless individuals age 62+. The LSS LOSP program serves primarily low income and formerly homeless adults with complex and long-term mental health, substance abuse, and medical issues. The support services team is comprised of a full-time case manager, a part time program Activities Coordinator, and support from the Adult and Senior Housing Services Program Manager. The team works together to ensure that residents are provided with necessary resources to ensure housing stability. A primary focus of the 735 Davis Apartments housing program is to support and inspire housing community members' independence and well-being through advocacy, linkage to resources, education, and other supportive services.


Essential Duties and Responsibilities:


  • Work collaboratively with the onsite Case Manager to conduct ageappropriate activities and events for resident adults, seniors, families.
  • Provide general counseling, referrals, resources, and other services as needed.
  • Fulfill administrative responsibilities, including daily attendance logs, monthly calendar of events, demographics and monthly reports, resident surveys, and program and office supply inventory (coordinating orders with the onsite Case Manager).
  • Provide light case management and/or social service coordination.
  • Assist in developing individual service plans related to resident outreach and engagement, in coordination with the onsite Case Manager.
  • Receive, prepare, and participate in weekly onsite Food Bank Distribution.
  • Create and coordinate program activities for all onsite residents.
  • Serve as liaison/advocate with outside social and health agencies; Provide current information and referral regarding services and community resources to residents and staff through case coordination, resident meetings, programming, staff meetings.
  • Observe client confidentiality & HIPAA protocols.
  • Maintain program's standard of documentation of services; keep accurate records, files, correspondence & statistics in both written form and via data entry in a timely manner.
  • Prepare reports & presentations of information as required by Supervisor and LSS Leadership.
  • Work in collaboration with other onsite staff including Property Management to facilitate community relations, provide guidance and conflict resolution meetings when needed to residents.
  • Evening hours are often required to assist with weekly and monthly community events.
  • Participate in regular trainings as assigned.
  • Perform other related duties as assigned by Supervisor and LSS Leadership.

Qualifications:

  • BA or BS degree from an accredited college or university, preferably with a major in a behavioral science, or a combined record of related education and lived experience.
  • Minimum 2 years' experience working with adults, seniors and families providing services to diverse populations.
  • Experience working with and sensitivity to issues affecting lowincome families and individuals.
  • Experience working with adults who have experienced chronic homelessness, severe mental health issues, substance use issues, and complex medical conditions.

Must demonstrate competent computer skills:
Windows, Microsoft Office, Outlook, Internet, etc.
