    Job Description: Here's what you'll be doing as a Social Worker 3:

    • Serve as social worker and/or case manager and perform mental health assessments of patients.
    • Gather necessary information to determine needs and establish treatment plans for patients.
    • Implement therapeutic techniques suited to patients' needs and monitor their progress.
    • Complete release of information forms with patients or their representatives.
    • Prepare discharge plans in collaboration with mental health centers, families, or representatives.
    • Liaison with social service agencies and institutions to assist patients.
    • Help patients develop community living skills.

    What We Offer:

    • Bachelor's Degree in Counseling and Guidance, Rehabilitation Counseling, Psychology, or a related field.
    • Minimum of two years of full-time experience in relevant settings.
    • Knowledge of social work principles, case management, and continuum of care.
    • Understanding of mental disabilities and related treatments.
    • Must obtain CPR and other relevant certifications.
