Essential Responsibilities / Job Duties

  • Ensuring all services provided are in accordance with standards established by various local, state and federal regulatory organizations, including management of a personal caseload of multiple Individuals participating in waiver services.
  • Directly supervises Direct Care Professional employees with authority to plan, assign, and direct tasks and responsibilities as needed and requested.
  • Attend and participate in team meetings, monitor individual progress, and provide detailed/accurate summaries on a quarterly basis.
  • Develop and update formal programs that support the wants, needs, and dreams of those served, as well as coordinate medical oversight with the organization, external partners, and healthcare providers.
  • Responsible for the management, training, and development of assigned Direct Care Professionals employees.
  • Responsible for timely generating, updating, and maintaining health, safety, and risk mitigation plans for Individuals served.
  • Provide supervision and training to all program personnel regarding the implementation of an Individual's plan and teaching strategies.
  • Perform routine Quality Inspections and coordinate programs in a way that meets all state and accrediting body expectations for services.
  • Submission and completion of incident reports as required in accordance with BDS standards, state and federal regulation, and organization policies.
  • Continuous review, oversight, and processing of documentation, including but not limited to daily notes, timesheets, goals, high risk or behavior tracking, and medication administration. Must be able to verify that information is accurate and consistent with the plan(s) for the Individual served.
  • Effectively initiate, coordinate, and conduct, any/all required trainings for employees and schedule updates as needed to remain current and within compliance.
  • Develop and maintain individual files per regulatory requirements and organization policies.
  • As needed and/or required, provide direct care support as needed to ensure and protect the health and safety of the individuals served.
  • Provide emergency oncall services as assigned and be able to respond in a prompt and appropriate manner.
  • Any/all other duties as assigned and required to meet the needs of those that the organization serves.

Qualifications

  • Must be at least 18 years of age.
  • Must have and maintain a valid Indiana Driver's License, an appropriately registered and insured vehicle, safe for travel and transport.
  • Satisfactory completion of criminal background checks as required by federal and/or state laws and any other postoffer, preemployment screenings as mandated by a governing organization or the organization.
  • Minimum of 1year prior working experience with Individuals with developmental, intellectual and/or physical disabilities

  • Must possess ability to effectively communicate verbally and through written communication.
  • Demonstrated ability to build and lead strong teams,
  • Should be a selfstarter and be comfortable working independently.
  • Must possess strong organizational, interpersonal, and decisionmaking skills.
  • Any equivalent combination of education and experience to provide the following Key Success Indicators (KSIs):
  • Ability to relate to and build relationships with service recipients, their families, providers, and others as required.
  • Ability to engage with Individuals in setting realistic and appropriate goals.
  • Ability to lead, direct and coordinate a team of care providers to meet the needs of Individuals.
  • Regulatory compliance with federal and state rules and regulations pertaining to the ID/DD population.
  • Valid driver's license, proof of automobile insurance, and reliable transportation.

Pay:
$45, $50,000.00 per year
