We are a Mobile Clinic that visits patient's home, board and care, assisted living facilities and skilled nursing.

• You will be assigned to a City of your choice or near your Home/ Work covering multiple locations and working with different teams to manage patients at their homes
• You will deliver proactive preventive care at patients' homes to reduce unnecessary hospitalizations visits.
• Partner with the patient's primary care provider and specialists to deliver integrated care
• Diagnose and treat patients, consistent with standard of care
• You will be conducting comprehensive exams and creating care plans for the most vulnerable, senior elderly Medicare patients
• Providing appropriate education and counseling to patients and their families/caregivers
• Documenting appropriately in EHR and other systems
• Collaborating with other members of the care team to manage the needs of the patient

This is a fun and rewarding type of Job. If you don't have time to do your online notes we can help you do it.

We have a medical transcriber standing by to help you so you can stay focused on your patient, family and friends at the end of the day.
