North Range Behavioral Health is a recipient of federal funding through the Substance Abuse and Mental Health Services Administration (SAMHSA) to be a Certified Community Behavioral Health Clinic (CCBHC).
The team includes clinicians, mental health counselors, and case managers working together to create a seamless and smooth front door experience for our large community behavioral health center in Weld County, Colorado.
The primary function of a clinician on this team is as an intake specialist.Duties Performed:
Provide person-centered assessment, goal planning, and treatment for mental health and/or substance use issues. Provide screening, intakes, and assessment of clients utilizing required instruments and formats within appropriate timelines. Develop strength-based written treatment plans/updates and continuing care plans and make appropriate referrals. Complete written discharge summaries and correspondence within established timelines. Observe clients by conducting intake interviews, in outpatient clinic and telehealth settings.
Assess mental status of clients, evaluate current level of functioning, and develop and implement treatment/service plans to address clients needs.
Intervene and de-escalate crises with clients.Provide crisis intervention via telephone or face-to-face in an outpatient setting Assess the risk of harm to self or others and arrange needed resources.
Collaborate with other clinicians, law enforcement, external providers, and psychiatrists to provide client services.Maintain close and cooperative relationships with other agency teams, community agencies and other public agencies in carrying out treatment/service plans.
Collaborate continuously with assigned clients to devise safety planning, provide symptom manag ment, identify support systems, and reduce and mitigate risk factors.
May deliver services to clients in their homes via telehealth or other settings in the community.Assist individuals in improving their well-being and quality of life by training in behavioral skills; educate clients to make informed choices; develop clients coping skills; help clients solve and negotiate daily living tasks.
Learn, utilize, and support fidelity of any evidence-based practices within the assigned program to ensure effective client interventions.Perform case management services, as needed, to assist clients to meet their goals as stated in client service and/or treatment plans.
Refer clients to internal and external supportive services such as vocational, medical, social services, and housing authority as well as residential, respite, and inpatient treatment.
Complete National Outcome Measures (NOMs) assessments at intake, reassessment, and discharge frequencies and other, as needed, assigned assessments to meet CCBHC grant compliance.
Maintain electronic health records following agency standards; confirm coding of services rendered is ethically appropriate and results in successful billing.
This may include entry into a partnering agency's electronic systems.Education/Credentials:
A Master s Degree in psychology, social work, or another behavioral health discipline from an accredited college or university is required.
