JOB DESCRIPTION

ECM Assistant Manager
HOUSING & SHELTER SERVICES


OVERVIEW:

Under the supervision of the ECM Manager. This is a lead position. Supervisory skills are expected. This position will require outreach, case management, tenant screening, assessments, and client advocacy. Assisting members who are Medi-Cal eligible. This position will also require the ability to track, document and report data.


ESSENTIAL FUNCTIONS:


  • Enhance Care Management for Medi-Cal members.
  • Work within the community with inperson interactions and outreach to impact high Medi-Cal users.
  • Conduct tenant screening and housing assessment that identifies the members preferences and barriers related to successful housing placement.
  • Identify and secure available resources to assist with subsidizing rent (work with HUD and Section 8 programs).
  • Manage a caseload of 20 Medi-Cal members.
  • Proficient with data entry, reporting, and data analysis.
  • Maintain digital records and member confidentiality.
  • Assist members with strategies to improve health outcomes.
  • Assist ECM Manager in supervising other ECM Case Managers.
  • Assist ECM Manager with onboarding and training.

EXPERIENCE/ KNOWLEDGE/ PHYSICAL REQUIREMENTS:


  • Ability to communicate, read, write, and follow oral and written instructions. Relay all communication effectively and professionally to all clients and to our partners within the community.
  • Ability to lead and guide staff and communicate effectively with Management.
  • This position requires a background check, drug test and physical.
  • Must have own reliable transportation with proof of vehicle insurance.
  • Valid California Driver's License.
  • Ability to meet the physical demands of this job; walk and lift to 10 15lbs. on a regular basis.
  • Ability to work independently.

EDUCATION/CERTIFICATIONS:

Bachelor's degree preferred or equivalent work experience in Social Services, Public Health, or Behavioral Sciences.
