Description
The Case Manager develops working relationships with customers and monitors engagement and progress.- Works with customers on a regular basis to meet and achieve program goals and/or maintain program services and eligibility
- Assists customers to acquire services that facilitate program goals (e.g., educational and/or vocational training, medical, child care, transportation, substance abuse/mental health, child support establishment, legal, and other related needs)
- Follows up with customers to ensure that their needs are met and questions and concerns are resolved
- Interacts with other team members to provide expertise and assistance in resolving participant issues
- Maintains accurate and timely case notes on all customer contacts and document activities
- Shares information about outreach and engagement efforts with project staff
- Informs project staff of any barriers that they identify and that are preventing the customer from engaging with the Provider
- Performs other duties as may be assigned by leadership
Educational/Experience Requirement
- Bachelor's Degree is required. In lieu of degree, four (4) years of experience in employment services or human services required.
Bilingual Capabilities in Arabic, Farsi, Spanish, or Vietnamese
Essential Duties/Responsibilities
- May have additional training or education in area of specialization.
- Works on assignments that are routine in nature, with responsibilities easily learned on the job.
- Acquires job skills and learns applicable policies and procedures to complete routine tasks.
- Able to read, understand & perform assignments within prescribed guidelines.
- Communicates routine information in a clear and accurate way with internal & external contacts.
Pay:
From $21.43 per hour
